Weatherspoon Plant (Weatherspoon Plant) is located at 491 Power Plant Road, Lumberton, North Carolina. In 1995, a leak in the 500,000-gallon above ground storage tank (AST) was discovered, which was a result of corrosion of the bottom of the tank. The tank has been fixed and is still in service. The subject site is an active power plant owned and operated by Duke Energy Progress, LLC (Duke Energy), formerly Carolina Power and Light (CP&L). The purpose of this report is to document the latest groundwater sampling event (conducted in June 2022) and free-phase product recovery efforts for 2022. Between June 28 and 29, 2022, CATLIN personnel conducted gauging and sampling activities on-site. Groundwater elevations in the shallow wells/piezometers ranged from 120.49 feet (MW-5) to 126.37 feet (P-53). The groundwater flow direction was towards the southeast, which is consistent with previous events. Free-phase product was measured in wells/piezometers RW-03, P-47, and P-81. Groundwater samples were collected from all monitoring wells and select piezometers and analyzed for Environmental Protection Agency (EPA) Method 602 compounds per Standard Method (SM) 6200B. Laboratory analysis revealed benzene and/or naphthalene concentrations above the Title 15A of the North Carolina Administrative Code, Subchapter 2L (2L GWQS) in samples collected from MW-3, P-53, P-68, P-70, T-71, P-75, P-77 and P-79. CATLIN evaluated the dissolved benzene and/or naphthalene trends at monitoring well MW-3 and piezometers P-53, P-70, T-71, P-75, P-77 and P-79 utilizing the GSI Mann-KendalL Toolkit for Constituent Trend Analysis (MK Toolkit). Based on the results from the MK Toolkit, dissolved benzene is decreasing in MW-3, P-75, P-77 and P-79. Naphthalene concentrations are decreasing in MW-3 and P-75. The benzene and naphthalene concentrations in the remaining piezometers were either stable or no trend was observed. A potable water supply well (WS-1) is located on-site approximately 350 feet west of the source area (in an upgradient direction). A non-potable well is located on site approximately 195 feet north northwest of the source area (in a side-gradient direction) that feeds the drip system that surrounds the source area. Well WS-1 has historically been sampled, but the drip system well has not. The June 2022 analytical results for the WSW-1 sample revealed all concentrations were below laboratory method detection limits. Free-phase product recovery efforts have resulted in the recovery of approximately 47 gallons of product for 2022 (as of June 2022). Duke Energy Progress; 197255_MonReport_June 2022_Draft2 CATLIN Engineers and Scientists CATLIN Project No. 197255 3 November 2022 3.0 PURPOSE CATLIN Engineers and Scientists (CATLIN) prepared this Groundwater Monitoring and Remediation System Report for Duke Energy. The purpose of this report is to describe the field procedures and monitoring results associated with the June 2022 monitoring event at the Weatherspoon Plant located in Lumberton, North Carolina. In addition, the report describes the corresponding system performance data and free-phase product recovery. 4.0 SITE HISTORY AND CHARACTERIZATION The Weatherspoon Plant site is located at 491 Power Plant Road, Lumberton, North Carolina (Figures 1 and 2). 4.1 AST OWNER AND OPERATOR INFORMATION Duke Energy has owned the site since 1949 and operated the ASTs since their installation in 1969. Available information regarding AST owners/operators is presented on Table 1. 4.2 AST INFORMATION Two ASTs (one 250,000-gallon and one 500,000-gallon) are currently in operation at the site. These tanks were installed in 1969 and contain No. 2 fuel oil. AST information is presented in Table 1. 4.3 PETROLEUM CONSTITUENT SOURCE INVENTORY Petroleum impacts exist on the Weatherspoon Plant as a result of a leak, discovered on March 16, 1995, due to corrosion of the steel bottom of the 500,000-gallon AST. According to a pore space volume estimate by Edwin Andrews and Associates, P.C. (Andrews & Associates), approximately 153,600 gallons of No. 2 fuel oil was released. 4.4 SITE CHARACTERISTICS The subject site is an active Duke Energy Power Plant and consists of several structures, equipment, cooling ponds and former coal ash ponds. The surface is a combination of paved and grassy areas. Two petroleum areas of concern (AOC) have been identified at the Weatherspoon Plant. AOC-1 (incident number 13884) is associated with the fuel oil release from the 500,000-gallon AST and is addressed in this report. AOC-2 (incident number 92156) is a suspected impacted soil area which was discovered during plant decommissioning activities. In 2019, AOC-2 was surveyed and sub-divided and a Notice of Residual Petroleum (NRP) was Duke Energy Progress; 197255_MonReport_June 2022_Draft2 CATLIN Engineers and Scientists CATLIN Project No. 197255 4 November 2022 recorded with the Robeson County Register of Deeds for this section of the Weatherspoon Plant. Appendix A provides a figure which identifies the AOC-1 and AOC-2 locations as well as the section of the property with the recorded NRP. 4.5 RECEPTOR INFORMATION Although CATLIN did not perform a formal receptor survey update during this monitoring period, potential receptors located within 1,500 feet of the source area are illustrated on Figure 2 and pertinent receptor information is described below. Water Supply Wells The Weatherspoon plant encompasses greater than a 1,500-foot radius around the source area. Currently, one active water supply well (WS-1) is located on the property approximately 350 feet west of the source area (in an upgradient direction). Based on conversations with Duke Energy employees, this well is a potable well and is the only source of water for the site. Additionally, the drip system, visible in Figure 3, is fed by a non-potable six-inch diameter well screened from 200 to 210 feet below land surface (BLS) located approximately 195 feet to the north northwest of the source area. Information regarding water supply wells within 1,500 feet of the site are detailed on Table 2. Public Water Supply Wells No public water supply wells were identified within 1,500 feet of the source area. Public water is not provided to the subject site. Surface Water Several surface water bodies were identified on-site. However, the only surface water body considered to be “waters of the United States” is Lumber River which is located approximately 1,300 feet west of the release. Wellhead Protection Areas On September 5, 2022, CATLIN accessed the NCDEQ, Division of Water Resources (DWR), Public Water Supply Well (PWS) Section web page and verified that there are no known wellhead protection areas within 1,500 feet of the subject site. Duke Energy Progress; 197255_MonReport_June 2022_Draft2 CATLIN Engineers and Scientists CATLIN Project No. 197255 5 November 2022 4.6 PREVIOUS INVESTIGATIONS AND REMEDIAL ACTIVITIES Summaries of previous investigations and remedial activities are provided below. July 1995 Site Assessment Report Following discovery of the leaking AST, Andrews & Associates prepared a Site Assessment Report in July 1995, which detailed repairs to the bottom of the AST. In addition, 27 piezometers were installed to help determine groundwater flow direction and help delineate the extent of the free-phase product. Based the piezometer gauging data, five shallow monitoring wells were installed between May 15, 1995 and June 2, 1995. January 1996 Corrective Action Plan A Corrective Action Plan (CAP) was prepared by Andrews & Associates, dated January 31, 1996. An estimated 40,000 gallons of No. 2 fuel oil was recovered as part of initial recovery efforts. These recovery activities involved the use of temporary recovery sumps that were later replaced by permanent recovery and drawdown wells. In the CAP, Andrews & Associates also detailed the proposed installation of an underground drip line to provide lateral and downgradient hydraulic control and enhance recovery efforts. In addition, Andrews & Associates recommended a semi-annual monitoring program, which the North Carolina Department of Health, Environment and Natural Resources (NCDHENR) – now the North Carolina Department of Environmental Quality (NCDEQ) approved. Corrective Action Implementation Subsequent site remedial activities, following the approval of the CAP, included the construction of the underground drip line in 1996. In 2004 and 2006 the drip line was expanded to the revised known extent of free-phase and dissolved-phase impact. Three additional recovery/drawdown wells were installed in 2008 within the known free-phase product plume, bringing the site total to six recovery/drawdown wells. The original recovery wells were 24-inch diameter and constructed utilizing corrugated high-density polyethylene (HDPE). Subsequently, 18-inch diameter PVC recovery wells were installed inside the 24-inch diameter HDPE pipe and the annular space was filled with sand. July 2014 Site Characterization Report In July 2014, CATLIN submitted a Site Characterization Report conducted by Columbia Technologies for Duke Energy Progress, LLC. As part of the site characterization, Laser Induced Fluorescence/Ultraviolet Optical Screening Duke Energy Progress; 197255_MonReport_June 2022_Draft2 CATLIN Engineers and Scientists CATLIN Project No. 197255 6 November 2022 Tool (LIF/UVOST) direct sensing, in conjunction with direct push technology (DPT), was employed to further define the horizontal extent of free-phase product. October 2014 Piezometer Installation CATLIN installed seven additional piezometers (P-68, P-69, P-70, T-71, T-72, T-73 and T-74) in October 2014. The piezometers were installed in areas of probable free-phase product accumulation (based on LIF results). Findings from the LIF investigation and subsequent free-phase product gauging events indicated the continued presence of interstitially trapped free-phase product. June 2015 Piezometer and Well Installation In June 2015, CATLIN installed 12 additional shallow piezometers to inspect for potential free-phase product accumulation; five temporary piezometers were installed within the 500,000-gallon AST containment area (for product gauging prior to the relining of the AST containment), and seven were installed in the vicinity of RW-00, RW-01, RW-03 and RW-05 (to be utilized for product gauging and recovery well testing). One Type III monitoring well (TMW-1) was installed to a depth of approximately 39 feet BLS to define the vertical limit of petroleum impacts. Following gauging activities, the five temporary piezometers within the 500,000-gallon AST containment area were abandoned. November/December 2015 Pumping Tests Between November 30, 2015, and December 31, 2015, CATLIN conducted 72- hour pumping tests on recovery wells RW-00, RW-01, RW-03 and RW-05 (one recovery well at a time). The overall results indicated that the free-phase product recovery and radial influence was likely being hindered by a combination of metal, mineral and biofouling surrounding the recovery wells and well cleaning was recommended. November/ December 2016 Well Rehabilitation A chemical well cleaning event was performed in November and December 2016. Recovery wells RW-01, RW-03, and RW-05 were each cleaned using a precise combination of Acid, Biocide and Surfactant along with an aggressive Surger and Gantry System to breakdown fouling both inside and outside the recovery well screen. Once all dosing and surging activities were completed, a vacuum truck was utilized to extract sediments and biofouling from within the recovery wells. Duke Energy Progress; 197255_MonReport_June 2022_Draft2 CATLIN Engineers and Scientists CATLIN Project No. 197255 7 November 2022 June 2018 Comprehensive System Evaluation Report A Comprehensive System Evaluation Report was submitted on June 15, 2018, which included all the activities conducted at the site from November 2015 to May 2018. These activities included the preliminary recovery well testing, the recovery well cleaning event, the post recovery well testing, and the long term free-phase product recovery evaluation. The report indicated that with aggressive well maintenance practices, free-phase product recovery could be enhanced; however, the long-term success of these efforts would be limited as a result of excessive biofouling and mineralization in the vicinity of the recovery wells and aquifer matrix. In addition, a dynamic pulse biosparge pilot test was recommended to address free-phase product potentially trapped in the soil matrix surrounding the recovery system and tank containment area. However, upon initial planning and evaluation, the pilot test was not scheduled due to AST foundation and structural integrity concerns. 4.7 CURRENT RISK AND LAND USE CLASSIFICATION Risk Classification criteria are defined in Title 15A Subchapter 02L .0506 of the North Carolina Administrative Code (15A NCAC 02L .0506). This site has been assigned a high-risk classification based upon the following: • A potable water supply well is located within 1,000 feet of the source area. • A non-potable water supply well is located within 250 feet of the source area. • The groundwater within 500 feet of the source area of a confirmed release has the potential for future use. • Free-phase product thicknesses greater than 0.01 feet are present at the site. Land Use Classification is described in 15A NCAC 02L .0408 (1). This site should be classified as industrial/commercial because exposure to the petroleum constituents is limited in time due to the use of the site and does not involve exposure to children. 4.8 REMEDIATION GOALS Cleanup goals applicable to the site and as presented in UST Section Corrective Action Guidelines are as follows: Soil—The current remediation goals for petroleum constituents in soil at high-risk sites are levels equal to or less than the lowest maximum soil contaminant concentrations (MSCCs). However, because of the shallow groundwater table (e.g., the seasonal high groundwater table is located Duke Energy Progress; 197255_MonReport_June 2022_Draft2 CATLIN Engineers and Scientists CATLIN Project No. 197255 8 November 2022 approximately at the ground surface), no petroleum-affected soil exists at the site outside the capillary fringe. Groundwater—Once free-phase product is removed, the remediation goals for groundwater at high-risk sites are levels equal to or less than the 2L GWQS. However, cleanup to alternate standards may be proposed if an alternate source of drinking water can be identified and provided to the area around the historical release location and groundwater use can be restricted. Surface Water—Surface water has not been affected by this release. Groundwater must be cleaned up to levels that are sufficient to prevent this site from causing a violation of the surface water rules contained in 15A NCAC 02B. Free-Phase Product—The remediation goal for free-phase product is no measurable amount of free-phase product greater than 0.01 feet in a groundwater monitoring well or piezometer. 5.0 SITE ASSESSMENT INFORMATION Between June 28 and 29, 2022, CATLIN personnel performed gauging and sampling activities on-site (refer to Figure 3 for site layout). CATLIN collected depth to groundwater table and free-phase product thickness data from site groundwater monitoring wells and select piezometers (refer to Table 1). CATLIN collected representative groundwater samples from shallow, Type II monitoring wells MW-1 through MW-5, telescoping well TMW-1, water supply well #1 (WS-1) and select piezometers. . 5.1 GAUGING DATA Depth to water was measured on June 28, 2022 using an electronic interface probe. To prevent cross contamination, the probe and cabling were decontaminated in accordance with manufacturer’s recommendations between use in each well. Gauging procedures were performed in general accordance with NCDEQ and EPA Region 4 guidelines. Groundwater measurements in the shallow site monitoring wells ranged from 2.52 feet (MW-2) to 5.43 feet (MW-1) BLS and groundwater elevations ranged from 120.49 feet (MW-5) to 125.43 feet (MW-1). The depth to water and groundwater elevation in deep monitoring well TMW-1 was 7.72 feet BLS and 120.92 feet, respectively. Depth to water in the piezometers ranged from 4.42 feet (P-46) to 8.49 feet (P-68) below top of casing (TOC) and groundwater elevations ranged from 124.38 feet (P-65) to 126.37 feet (P-53). The June 2022 gauging data is provided in Table 3. Duke Energy Progress; 197255_MonReport_June 2022_Draft2 CATLIN Engineers and Scientists CATLIN Project No. 197255 9 November 2022 CATLIN utilized the groundwater measurements to generate groundwater table contours (see Figure 4). Based on the June 2022 gauging data, the surficial groundwater flow is generally towards the southeast. Free-phase product was detected in recovery well RW-03 (0.76 feet) and piezometers P-47 (0.03 feet) and P-81 (0.31 feet). The June 2022 free-phase product gauging data is provided in Table 3 and historical free-phase product gauging data is provided in Table 4. The June 2022 free-phase product gauging data is illustrated on Figure 5. 5.2 SAMPLING DATA Following gauging, the monitoring wells and select piezometers were sampled in accordance with NCDEQ guidelines and U.S. EPA Region 4 low- flow operating procedures. New polyethylene tubing was inserted into each well/piezometer to a selected depth, based on the historical LIF data in the vicinity of the well/piezometer. The polyethylene tubing was connected with silicone tubing to a peristaltic pump that pumped water from the monitoring well to a flow through cell. The silicone tubing in the pump was replaced between each well to prevent possible cross-contamination. Attached to the flow through cell was a multi-parameter water quality instrument (YSI 556 MPS) that measures groundwater field parameter data including temperature, pH, conductivity, dissolved oxygen (DO), and redox potential (ORP). Groundwater field parameter data was collected at five-minute intervals. Monitoring well drawdown was monitored during sampling utilizing the electronic interface probe to ensure minimal depression of the water table and that formation fluids were collected for analysis. Temperature, pH, conductivity, dissolved oxygen, and ORP were selected as parameters for determination of groundwater stabilization. Once these selected parameters were within the desired ranges for three consecutive measurements, the flow through cell was disconnected and samples were collected directly from the peristaltic pump tubing (changed between wells) to the appropriate laboratory provided glassware for laboratory analysis. Low flow field measurement data sheets can be found in Appendix B. The water supply well sample (WS-1) was collected after opening the sample collection valve on the well and letting it run for approximately five minutes prior to sample collection. The groundwater monitoring well, piezometer and water supply well samples were transported to Pace Analytical Services, Inc. of Huntersville, North Carolina under proper Chain-of-Custody protocol for analysis per EPA Method 602, including Naphthalene. Duke Energy Progress; 197255_MonReport_June 2022_Draft2 CATLIN Engineers and Scientists CATLIN Project No. 197255 10 November 2022 5.3 SUMMARY OF ANALYTICAL RESULTS CATLIN collected a total of 19 groundwater samples from six monitoring wells (five shallow and one deep), 12 piezometers and one water supply well and submitted the samples for laboratory analysis for the June 2022 sampling event. Benzene concentrations were above the 2L GWQS (1 microgram per liter [µg/L]) in the groundwater samples collected from P-68 (15.5 µg/L), P-70 (2.7 µg/L), T-71 (16.0 µg/L), P-75 (2.7 µg/L), P-77 (2.2 µg/L), and P-79 (8.6 µg/L). Naphthalene concentrations were above the 2L GWQS (6 µg/L) in the groundwater samples collected from MW-3 (11.5 µg/L), P-53 (13.5 µg/L), P-68 (84.6 µg/L), P-70 (17.1 µg/L), T-71 (42.0 µg/L), P-75 (26.6 µg/L), P-77 (51.4 µg/L), and P-79 (28.0 µg/L). No other EPA Method 602 compound concentrations were above their corresponding 2L GWQS. The concentrations of constituents in sample WS-1, which was analyzed according to EPA Method 602, were below laboratory reporting limits. Laboratory results are summarized in Table 5 and illustrated on Figures 6 (Benzene) and 7 (Naphthalene). The laboratory analytical report, along with chain-of-custody documentation, has been included in Appendix C. Historical groundwater laboratory data per EPA Method 625 Base Neutral Extractables from June 2005 to January 2009 is summarized in Table 6. Historical groundwater laboratory data per EPA Method 602, including Naphthalene, from June 2009 to the present is summarized in Table 7. 5.4 STATISTICAL ANALYSES CATLIN used the Mann-Kendall statistical method to evaluate the dissolved benzene and/or naphthalene trends at MW-3 and piezometers P-53, P-70, T- 71, P-75, P-77 and P-79. CATLIN used the MK Toolkit to perform the statistical analysis. Using the MK Toolkit, CATLIN analyzed the groundwater monitoring data to quantitatively determine if the measured concentrations are increasing, decreasing or stable over time. Based on the results from the MK Toolkit, dissolved benzene concentrations in MW-3, P-75, P-77 and P-79 are decreasing, the concentration in P-70 is stable and there is no concentration trend in T-71. Dissolved naphthalene concentrations are decreasing at MW-3 and P-75, and concentrations are stable in P-53, P-70, T-71, P-77 and P-79. The MK Toolkit results are provided in Appendix D. 6.0 FREE-PHASE PRODUCT RECOVERY SYSTEM Currently, the recovery system consists of six recovery wells with groundwater pumps deployed in wells RW-01 and RW-03. The groundwater pumps facilitate the movement of free phase product toward each recovery well. Free-phase product in the recovery wells is removed manually with an air operated diaphragm pump. Product is transferred via the diaphragm pump to 55-gallon drums with secondary containment until a sufficient volume is available for pickup and recycling. Duke Energy Progress; 197255_MonReport_June 2022_Draft2 CATLIN Engineers and Scientists CATLIN Project No. 197255 11 November 2022 Recovered groundwater is pumped through a series of three polyethylene tanks and routed to the plant’s oily water recovery system where it undergoes further separation prior to discharge under NPDES permit NC0005363. The wells are cycled into or out of service, depending on the volume of recoverable free product present. Free-phase product recovery efforts are on-going at the site. Appendix E provides annual and cumulative totals of free-phase product recovered since 1995. In addition, the graph included in Appendix E illustrates the free-phase product recovery efforts. As of the June 2022 sampling event, the free-phase product recovery system has collected approximately 125,059 gallons of free-phase product. Since pumps were replaced in the recovery wells following the chemical well cleaning (February 2017), a total of approximately 1,277 gallons of free-phase product has been recovered from RW-01 and RW-03 as of June 29, 2022 (including approximately 47 gallons recovered so far in 2022). During the February 15, 2022 site visit, the pump in RW-01 was not operating properly. It was determined that the discharge hose was filled with sand. It appears that sand in the well was pumped through the discharge hose and also damaged the pump. The pump was replaced in June 2022. 7.0 ADDITIONAL SITE ACTIVITIES On March 31, 2022, the system free-phase product recovery system was shut off due to a high-level float switch malfunction in the last holding tank in the system. New cable for the high-level float switch was installed through a conduit and the system was re-started in June 2022. During the June 2022 sampling event, piezometers P-04 and P-76 were repaired without changing the TOC elevation. CATLIN repaired P-49 and surveyed the new TOC elevation (130.93 feet). During the site visit, P-42 could not be located and therefore, the TOC was not repaired. 8.0 CONCLUSIONS Updated groundwater findings from the June 2022 monitoring event are summarized below: • Groundwater flow is towards the southeast. • CATLIN measured free-phase product in wells/piezometers RW-03, P-47, and P-81. • Benzene and/or naphthalene concentrations are above the 2L GWQS in the groundwater samples from wells/piezometers MW-3, P-53, P-68, P-70, T-71, P-75, P-77 and P-79. The well is in an upgradient direction and all petroleum constituents are below laboratory reporting limits. 9.0 LIMITATIONS The groundwater samples analyzed as part of this investigation only provide isolated data points and may not represent conditions at every location in the project area. Analyses and conclusions of this report, being based on interpolation between data points at the project area, may not be completely representative of all site conditions. Conclusions and recommendations of this investigation and report are based on the best available data in an effort to comply with current regulatory requirements.
