EXECUTIVE SUMMARY
Former OM Mart is located at 113 West Main Street in Biscoe, Montgomery County, North Carolina. The
site is currently owned by VISHALP, LLC and is operated by Quality Oil & Gas as a petroleum retail facility,
tire service and convenience store. A site check was conducted in November 2011 following a failed line
test that confirmed the presence of a release. Currently, one 6,000-gallon gasoline, two 4,000-gallon
gasoline, one 2,000-gallon gasoline USTs, one kerosene AST, associated dispensers and product piping are
active at the site.
The site contains one building and is primarily capped with concrete and asphalt. The overall topography
at the site is relatively flat. No surface water bodies were identified within a 500-foot radius of the site.
A Phase I LSA was conducted in January 2008 by a past consultant. There are four unused/irrigation wells
within 1,000 feet of the site. The Town of Biscoe well identified during January 2008 receptor survey
activities is reportedly out of service and has been capped off. Municipal water serves all properties within
1,000 feet of the site. Based on this information and the past presence of ground water contamination
exceeding the GCLs, the site risk ranking is intermediate.
On November 16, 2022, two Type II monitoring wells (MW-12 and MW-13) were installed off-site to total
depths of 35 feet BGS, with screened intervals of 15 feet, each.
On November 16, 2022, twelve Type II monitoring wells were gauged, purged and sampled in accordance
with NCDEQ guidelines. Concentrations of individual BTEX constituents, MTBE, naphthalene, TAA, TAME,
bromodichloromethane, TBA, chloroform, 1,2 dichloroethane, isopropylbenzene, di-isopropyl ether, n-
propylbenzene, IPE, chlorobenzene, methyl chloride, p-dichlorobenzene, methylene chloride, 1,2,4-
trimethylbenzene and/or 1,3,5-trimethylbenzene that exceeded the MACs were reported in the ground
water samples collected from MW-1, MW-2, MW-5 and MW-7 through MW-12. The concentrations of
benzene reported in the ground water samples collected from MW-1, MW-7, MW-8 and MW-9 exceeded
the GCLs.
The November 16, 2022, sampling event will be used as the baseline for future contaminant mass
calculations.
Based on this information, the risk classification for the site should remain intermediate risk. The ground
water contaminant plume is reasonably defined for an intermediate risk site. Prior to developing a CAP
for this site, soil sampling should be conducted in the area of MW-1, MW-7, MW-8 and MW-9 to
determine the vertical and horizontal extent of soil contamination. Semi-annual ground water monitoring
should be continued at the site in order to establish contaminant mass trends. The next sampling event
is tentatively scheduled for May 2023.

1.0 SITE HISTORY AND CHARACTERIZATION
GRI presents the results of the November 2022 ground water sampling event at the Former OM Mart site;
located at 113 West Main Street in Biscoe, Montgomery County, North Carolina (Figure 1). These activities
were conducted in accordance with GRI Proposal No. 22-451, which was submitted to NCDEQ on June 20,
2022, and was approved as Task Authorization No. 29478-18 on June 21, 2022. On July 1 and November
8, 2022, change orders were submitted to NCDEQ and were approved on July 5 and November 9, 2022,
respectively. State correspondence has been included as Appendix A.
The site is currently owned by VISHALP, LLC and is operated by Quality Oil & Gas as a petroleum retail
facility, convenience store and tire service. A Phase II ESA was conducted at the site on September 26,
2007. Soil and groundwater contamination were both detected exceeding applicable regulatory
standards. Currently, one 6,000-gallon gasoline, two 4,000-gallon gasoline, one 2,000-gallon gasoline
USTs and one kerosene AST are active at the site. UST owner/operator information and UST system
information are included as Tables 1 and 2, respectively. The Registered Tank Report is shown below:
The site and surrounding area are a mixture of multi-family residential, single family residential, and
commercial properties. The site contains one building and is primarily capped with concrete and asphalt.
The overall topography at the site is relatively flat. No surface water bodies were identified within a 500-
foot radius of the site. Eleven Type II monitoring wells (MW-1 through MW-11) were installed at the site
during previous ground water assessment activities. A Site Map showing the locations of the monitoring
wells and the structures on-site has been included as Figure 2.
A Phase I LSA was conducted by the previous consultant in January 2008. There are four unused/irrigation
wells within 1,000-feet of the site. The Town of Biscoe well identified during January 2008 receptor survey

1,000 feet of the site. Based on the results of the receptor survey, as well as the past presence of ground
water contamination above the GCLs, the site risk ranking is intermediate.
On November 16, 2022, two Type II monitoring wells (MW-12 and MW-13) were installed off-site to total
depths of 35 feet BGS, with screened intervals of 15 feet, each. Well construction information is
summarized in Table 3. The locations of the monitoring wells are shown on Figure 2. Copies of the well
permits, well construction records and subsurface logs are included as Appendix B.
2.0 PRESENTATION OF CURRENT SITE ASSESSMENT INFORMATION/ COMPARISON TO HISTORICAL
ASSESSMENT INFORMATION
On November 16, 2022, twelve Type II monitoring wells MW-1, MW-2, MW-3 and MW-5 through MW-13
were gauged, purged and sampled in accordance with NCDEQ guidelines. Please note, MW-4 could not
be located. Depths to ground water in the Type II monitoring wells ranged from 17.22 feet to 24.25 feet
below the tops of well casings. Ground water elevations in the Type II monitoring wells ranged from 77.83
feet to 82.78 feet. Ground water flow is generally toward the west/southwest. The horizontal gradient
across the site is approximately 0.02 feet per foot. The vertical gradient at the site cannot be calculated.
Free product was not observed. Well construction and gauging information is presented in Table 3. A
Water Table Surface Map based on the November 2022 gauging event is included as Figure 3. A summary
of the current ground water gauging data is shown below:

Ground water samples collected from the monitoring wells were submitted for laboratory analyses of
VOCs plus 5 oxygenates using EPA Method 6200B. Concentrations of individual BTEX constituents, MTBE,
naphthalene, TAA, TAME, bromodichloromethane, TBA, chloroform, 1,2 dichloroethane,
isopropylbenzene, di-isopropyl ether, n-propylbenzene, IPE, chlorobenzene, methyl chloride, p-
dichlorobenzene, methylene chloride, 1,2,4-trimethylbenzene and/or 1,3,5-trimethylbenzene that
exceeded the MACs were reported in the ground water samples collected from MW-1, MW-2, MW-5 and
MW-7 through MW-12. Concentrations of benzene that exceeded the GCLs were reported in the ground

water samples collected from MW-1, MW-7, MW-8 and MW-9. A summary of the analytical results for
ground water samples collected on November 17, 2022, is presented in Table 4. A Ground Water Quality
Map based on the November 2022 data is included as Figure 4. A contaminant specific isoconcentration
map for benzene has been included as Figure 5. A complete laboratory report has been included as
Appendix C. A summary of the current ground water analytical data is shown below:

4.0 LIMITATIONS
This report has been prepared for the exclusive use of Quality Oil & Gas for specific application to the
referenced site in Montgomery County, North Carolina. The assessment was conducted based on the
scope of work and level of effort desired by NCDEQ and with resources adequate only for that scope of
work. Our findings have been developed in accordance with generally accepted standards of
environmental practices in the State of North Carolina, available information, and our professional
judgment. No other warranty is expressed or implied.
The data that are presented in this report are indicative of conditions that existed at the precise locations
sampled and at the time the samples were collected. In addition, the data obtained from samples would
be interpreted as being meaningful with respect to parameters indicated in the laboratory report. No
additional information can logically be inferred from these data.

The UST Section, Division of Waste Management, Fayetteville Regional Office, is in receipt of the
Ground Water Monitoring Report dated May 12, 2022. The UST Section agrees with the recommendation for
continued quarterly ground water monitoring in advance of preparation of a Comprehensive Site
Assessment Report. However, this office requests that you also include off-site MW-4 & MW-6 for at least the
next sampling event. The requested lab method 6200B should include Ethanol plus Oxygenates. The
next sampling event is anticipated for July 2022 with the data presented in the Monitoring Report format
during August 2022.
Also, because this parcel was sold during July 2019 and ground water contamination has not been
remediated to below “unrestricted use standards”, you were required by the North Carolina General Statute
(NCGS) 143B-279.9 and 143B-279.11 to file a Notice of Residual Petroleum (Notice) with the Register of
Deeds in Montgomery County and then to submit a certified copy of the filed Notice to the Fayetteville Regional
Office. This task should be completed as soon as possible to get into compliance with NCGS 143B-279.9 and
143B-279.11.
