The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Reviewed semi-annual groundwater (October 2017, May 2018, October 2018, April 2019, October 2019, April
2020, October 2020, April 2021, October 2021, May 2022 and November 2022) and monthly methane monitoring
reports continue to be performed by facility staff.
2. A gate is located at the entrance to the facility to prevent unauthorized entry.
3. Edge of waste markers were in place and visible along the perimeter of the landfill.
4. The landfill was last mowed during the summer months. An inspection of the landfill to inspect for any erosion
issues, depressions and leachate seeps was not performed due to the tall established vegetative cover. Facility staff
is scheduled to mow in the next month. Upon completion, an additional inspection will be performed of the landfill.
5. An inspection of monitoring wells was performed to verify proper tags, locks, and concrete pad. All wells were
locked and tagged. Several concrete bases were covered with debris. Mr. Bivins stated when the landfill is mowed
all debris will be removed from the bases. Monitoring well MW-7 had a broken hinge which allowed the cap to be
lifted. Mr. Bivins stated he would have staff immediately make the repairs to the hinge.
6. Temporary disaster debris site (DS55-005) is located southwest of the closed landfill and is still available for use.
