The Permit to Construct and Operate will expire on September 6, 2027. This permit is the combination of two former notified LCID landfills, which were less than 2 acres each. The northern LCID began operating in 2006 with the notification number of N0712.The southern LCID began operating in 2018 with the notification number ofN1096. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None Page 1 of 3 NORTH CARnLINAD_E Q�� OepaNnent W Environnnnbl puel� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. New signage has been placed at the entrance to the facility. Mr. Collie has also installed additional signage throughout the facility to aid in the operation of the LCID. 2. A gate has been installed at the entrance to the facility. 3. The sediment basin appeared to be functioning properly. Four fabric baffles run across the sediment basin. No overgrown vegetation was observed on the side slopes of the sediment basin. Continue to monitor and maintain the basin as required by the stormwater permit. 4. The access road to the facility is being managed properly at this time. An access road has also been established around the entire LCID to provide access for fire -fighting vehicles. The portion of the roadway near the southeastern corner of the LCID was wet and passage for vehicles would likely be difficult. Ensure that the access road around the LCID is of all weather construction. Repair the roadway as needed to allow for safe passage. 5. At the time of this inspection, the on -site attendant was moving and compacting waste near the working face. The working face was small and compact. Cover soil has been staged adjacent to the working face. The stand of pine trees between the previous Notified LCIDs, has been removed and was identified to be the location of the working face. Ensure that the working face is covered with no less than 6 inches of soil monthly or when the working face reaches one acre in size. 6. Exposed waste and erosion rills were identified on the southern portion of the LCID (former N1096), near the sediment basin. The waste must be covered with no less than 6 inches of soil. The erosion rills should be repaired and seeded to prevent further erosion. 7. Edge of waste markers have been installed around the LCID and can be seen from one to the next. Continue to maintain the edge of waste markers to ensure that they are visible around the LCID. 8. Two stockpiles of broken concrete were observed at the facility. Based on previous conversations with Mr. Collie, the concrete will be utilized as road base at the facility. 9. The eastern side slope of the northern portion of the LCID (former N0712) was identified to be steeper than 3 horizontal to 1 vertical. The side slopes of the former N0712 need to be reworked to meet the 3:1 ratio. 10. Photographs were taken during this inspection.
