Dear Mr. Overbey:
The NC Division of Waste Management has reviewed your application to renew Septage Detention and
Treatment Facility permit, SDTF-41-20, in Guilford County. Your application has been approved in
accordance with NC Septage Management Rules and your permit, SDTF-41-20, is enclosed. This permit
covers the use of ten buried concrete tanks for the storage of domestic septage and grease septage
(including the passive separation of grease septage). This facility consists offour— 1,000 gallon tanks, five
— 1,200 gallon tanks, and one — 20,000 gallon tank. This permit shall expire on April 13, 2032. An
application for permit renewal must be submitted ninety (90) days prior to permit expiration.
Please read the permit conditions carefully. When communicating to the Division about this permit, refer
to it as "SDTF-41-20." Failure to operate the septage detention facility in accordance with NC Septage
Management Rules and the enclosed permit could subject you to administrative penalties of up to
$15,000 per violation per day. If you have questions concerning this permit or septage in general, please

CONDITIONS OF OPERATING PERMIT
1. This facility shall be maintained and operated in a manner that will protect the
assigned water quality standards of both surface and ground waters.
2. Leaks and spills are the responsibility of the Permittee and shall be immediately
contained, cleaned up, and treated to prevent any contamination of the surrounding
area. Any solids from leaks or spills shall be disposed of in a manner consistent with
the NC Solid Waste Management Rules.
3. The method for controlling odors described in the approved application for this permit
must effectively control all odors created by the operation of this facility as required
by 15A NCAC 13B .0838(g).
4. Erosion and runoff control measures shall be implemented to prevent migration of
soil or wastes off this facility. The installation of groundwater monitoring wells or a
leak detection system may be required at the discretion of the Division of Waste
Management.
5. This permit shall not affect the applicability of any statutes, rules, regulations, or
ordinances that may be imposed by other local, state, or federal government agencies
that may have jurisdiction over this facility.
6. This permit may be modified or reissued at any time to incorporate any conditions,
limitations, and/or monitoring requirements the Division deems necessary to
adequately protect the environment and public health.
7. This facility has a total capacity of 30,000 gallons for the storage of domestic septage
and grease septage (including the passive separation of grease septage). The facility
consists of ten buried concrete tanks: four — 1,000 gallon tanks, five — 1,200 gallon
tanks, and one— 20,000 gallon tank. Storage or treatment of any liquid or solid wastes
other than those specified in the approved application for this permit is prohibited.
Liquid and solid waste generated at this facility shall be disposed of in a manner
consistent with the approved application and submitted authorizations.
Authorizations for liquid and solid waste disposal shall be submitted to the Division
prior to utilizing a disposal location. This office shall be contacted immediately if a
disposal location revokes the authorization.

CONDITIONS OF OPERATING PERMIT
This facility shall be maintained and operated in a manner that will protect the
assigned water quality standards of both surface and ground waters.
2. Leaks and spills are the responsibility of the Permittee and shall be immediately

area. The transfer of septage to and from the detention system shall be conducted
in a manner that prevents leaks or spills of septage as stated within Rule 15A NCAC
13B .0838(j). Any liquid or solids collected from the spillage or leakage of septage
shall be disposed of in a manner consistent with the NC Solid Waste Management
Rules. Major spills shall be reported to the Division of Waste Management within
24 hours of occurrence.
3. The method for controlling odors described in the approved application for this
permit must effectively minimize odors created by the operation of this facility as
required by 15A NCAC 13B .0838(g). Also, this facility shall be designed,
constructed, and maintained in such a manner as required by 15A NCAC 13B
.0838(e)(2), to minimize the attraction or admittance of vectors.
4. Erosion and runoff control measures shall be implemented to prevent migration of
soil or wastes off this facility. The installation of groundwater monitoring wells or a
leak detection system may be required at the discretion of the Division.
5. This permit shall not affect the applicability of any statutes, rules, regulations, or

7. Storage or treatment of any liquid or solid wastes other than domestic septage
(including portable toilet waste), grease septage and commercial/industrial
septage as specified in the approved application for this permit is prohibited. This
septage treatment facility (to include one concrete screening tank and one
dewatering tank) consists of one 4,000 reclaim water tank and two 10,000 gallon
storage tanks with a total capacity of 24,000 gallons for the storage of untreated
and treated septage. Liquid and solid waste generated at this facility shall be
disposed of in a manner consistent with the approved application and submitted
authorizations. Authorizations for liquid and solid waste disposal shall be submitted
to the Division prior to utilizing a disposal location. This office shall be contacted
immediately if a disposal location revokes the authorization.
8. No one other than the Permittee or an employee of the firm named in this permit
may discharge septage at this facility unless the Division is notified and approval is
granted by the Division. Only Septage Management Firms permitted to operate in
North Carolina may receive authorizations to discharge at this facility. Each Firm
(other than the Permittee) shall have a properly completed and approved
"Authorization to Discharge Septage at a Septage Treatment or Storage Facility
Permitted to Someone Other than Yourself" on file with the Division before using
this facility.
9. This permit shall expire on August 31, 2023. Renewal or modification of this permit
shall be in accordance with 15A NCAC 13B .0837 (i), (m), (n), or (q). An application
for permit renewal shall be submitted at least ninety (90) days prior to the permit
renewal date. Accurate site drawings (2 copies with a North Carolina PE stamp), a
signed & dated operation and maintenance manual (revised to reflect any physical
or operational changes at the facility), current solids and wastewater disposal
authorizations and a copy of the required facility records shall be submitted with
the renewal application.
10. The storage capacity, construction, treatment methods, and operation plan of this
facility shall be as represented in the permit application. Any changes shall require
a permit modification.

11. Records shall be maintained in accordance with the Septage Management Rule 15A
NCAC 13B .0838(v). These records shall be made available to a representative of the
Division upon request. A copy of these records for the period of time this permit
was valid shall be submitted along with an application for permit renewal or
modification.
12.Any duly authorized officer, employee, or representative of the Division may, upon
presentation of credentials, enter and inspect any property, premises, or place on or
related to the facility, at any reasonable time for the purpose of determining
compliance with this permit; may inspect or copy any records that must be kept
under the conditions of this permit; or may obtain samples of groundwater, surface
water, or leachate.
13.The dewatering box shall be cleaned as described in the operation and maintenance
manual and in such a manner that all rinsate and solids are contained and disposed
of properly.
14. The setbacks prescribed by 15A NCAC 13B .0838(m) shall be maintained at all times.
