The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 - This site visit was conducted as a Pre-Operation’s Inspection to ensure the Vertical Expansion of Fill Sequence 2 meets the requirements of the Permit To Construct (PTC) as is written in the current Permit To Operate (PTO) dated April 12, 2018. New PTO will be issued upon this inspection.
02 -This permit approves construction of the 0.5-acre expansion area at the southeast corner of the landfill unit (also referred to as fill sequence 2).
03 -The required groundwater (GW) well has been installed and was locked and tagged as required. Christine Ritter (NCDEQ-DWM-SWS-Environmental Engineer) has approved the use of this well for GW comparison sampling.

04 - The Permanent Edge of Waste Markers have been installed for this expansion.
05 - The Berm outside of the Edge of Waste was put to a test. Approximately, a 1.0 inch rain event occurred the day/night before this inspection.
06 - No major erosion was noted on the berm. The top right corner of the above picture shows water ponded. The berm is higher than the water level. 07 - All water MUST be removed before any waste is placed into this expansion area. 08 - There has been a tile put into the surface water drainage ditch with a crushed inert debris road going to the new expansion of this site.

09 - Small samplings need to be removed from side slope in the background of the picture above.
