Good afternoon Courtney,
I hope you are well. The Solid Waste Section is in receipt of the application submitted on
January 3, 2023 for a proposed Land Clearing and Inert Debris Landfill in Rockingham
County. The attached compliance history review is a standard request during the permitting
process, which asks for submittal of an organization chart and completion of a compliance
questionnaire. The Section will not be able to complete the permit review process without the
requested information in this email under Environmental Compliance Requirements.

Please submit an organization chart at the corporate level that reflects all business entities that
are direct or indirect parents, subsidiaries, or other affiliates of Thomas Stanley Grading and
Hauling, Inc..
Additionally:
• Depict the relationships among all entities shown on the chart; as well as any
partnerships and joint ventures in which any of the entities have a financial interest.
• For all business entities identified in the organizational charts, please identify those
business entities which operated under a previous business name and identify the prior
business name.
• Include any existing or expired State issued permit numbers associated with the business
entities.

(NCGS 130A-294(b2)).
