The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) This facility consists of one 5,000-gallon, three 20,000-gallon, and one 30,000-gallon tanks for the
storage and treatment of domestic septage, portable toilet waste, grease trap pumpings, and
commercial/industrial septage. All five tanks are above ground metal tanks. The 20,000-gallon tank has
failed, and Mr. Bean stated that this tank will be replaced with a tank of the same material and size.
2) The access road is well maintained as required per 15A NCAC 13B .0838(b).
3) Holes or cracks which may allow the escape of septage were noted as required per 15A NCAC 13B
.0838(c).
4) No vectors were noted at the site, complying with 15A NCAC 13B .0838(e)(2).
5) No odors were noted at the site, complying with 15A NCAC 13B .0838(g).
6) No vegetation was noted within 5 feet of tank, as required per 15A NCAC 13B .0838(i).
7) No trespassing signs were present at the entrance to the site, complying with 15A NCAC 13B .0838(k).
8) Setback requirements appear to have been met per 15A NCAC 13B .0838(m).
