OBSERVED VIOLATIONS: N/A The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Page 1 of 2 FACILITY COMPLIANCE INSPECTION REPORT D_E Q�� Division of Waste Management NORTH CAROLINA Ow-- W Emironm l puelily Solid Waste Section ADDITIONAL COMMENTS 1) This facility consists of one 1,500 gallon concrete tank and one 3,500 gallon concrete tank for storage and treatment of domestic septage, portable toilet waste, and grease trap pumpings. 2) Setback requirements appear to have been met. At the time of inspection, vegetation was present within 5-feet of the tanks, not meeting the requirement of Rule 15A NCAC 13B .0838(i). Following the inspection, Mr. Mecimore had the vegetation cleared surrounding the tanks. 3) Appropriate signate was observed at the entrance to the site. 4) The access road was adequately maintained to meet the requirements of Rule 15A NCAC 13B .0838(b). 5) No holes or cracks which may allow the escape of septage out of the tanks were observed. The tanks appeared to be in compliance with Rule 15A NCAC 13B .0838(c). As noted on the previous page, the repair of a leak previously noted appears to be in tact. 6) Septage appears to be transferred to and from the tanks in a manner that prevents leaks or spills of septage onto the tank or ground as per 15A NCAC 13B .0838(j). 7) No vector or odor issues were noticed as covered within Rules 15A NCAC 13B .0838(e)(2) and .0838(g).
