Box 728 Kernersville, NC 27284 PARTICIPANTS: Greg Ray – City of Kernersville Brad Beeson, Solid Waste Superintendent – City of Kernersville Mathew James, Project Geologist – Withers Ravenel Robert Pike, Environmental Senior Specialist – Solid Waste Section STATUS OF PERMIT: Closed PURPOSE OF SITE VISIT: Post-closure Inspection STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: A. 15A NCAC 02C .0108(m) states “All non-water supply wells shall be equipped with a steel outer well casing or flush-mount cover, set in concrete, and other measures to protect the well from damage by normal site activities.” The Town of Kernersville is in violation of 15A NCAC 02C .0108(m) for failure to have all monitoring well steel outer well casings set in concrete. During the inspection, monitoring wells GW-3, MW-3S, and MW-3D were observed having damaged and missing sections of the concrete pad surrounding the steel outer well casings. To achieve compliance, the Town of Kernersville must repair the damage to the concrete pad for GW-3, MW-3S, and MW-3D. B. 15A NCAC 02C .0108(p) states “Each non-water supply well shall have permanently affixed an identification plate. The identification plate shall be constructed of a durable, waterproof, or rustproof material and shall contain the following information: (1) well contractor's name and certification number; (2) the date the well was completed; (3) the total depth of the well; (4) a warning that the well is not for water supply and that the groundwater may contain hazardous materials; (5) the depth to the top and bottom of each screen; and (6) the well identification number or name assigned by the well The Town of Kernersville is in violation of 15A NCAC 02C .0108(p) for failing to have all groundwater and landfill gas monitoring wells properly labeled with the required information. During the inspection, monitoring wells MW-3S, MW-3D, and GW-3 had permanently affixed identification metal plates but did not have the required data included on the plates. To achieve compliance the Town of Kernersville must label the affixed metal plates with the information as specified in 15A NCAC 02C .0108(p). ADDITIONAL COMMENTS 1. All pictures included in this report were taken by Robert Pike on November 29, 2022. 2. The facility is a closed unlined sanitary landfill. The facility is landfill is located on the south side of Freeman Road at the Guilford County line. All of the permitted landfill facility is located within Forsyth County. 3. The facility is secured by a perimeter chain link fence with access controlled by a locking gate. 4. The facility roadways were of all-weather construction and in good condition. 5. The facility conducts groundwater sampling semi-annually and conducts gas sampling monthly as required. 6. Edge of waste markers were observed on the landfill. Edge of waste markers were located at the top of the landfill slopes. 7. The cap of the landfill was covered with well-established and maintained vegetative cover. The landfill cap had The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Ensure that maintenance is performed to fill in the depressed area with clean soil to prevent the impounding of water on the landfill cap. 8. All of the environmental monitoring wells were observed during the inspection. All monitoring wells were accessible and secured with a lock. However, some deficiencies were observed during the inspection. (See the Observed Violations section of this report for additional information.)
