None.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The facility is permitted to receive, consolidate, and transfer yard trash. The site receives and manages leaves
collected curbside by the city of Winston-Salem Sanitation Division. The site receives and stores the leaves through the months of November through March. 2. No leaf grinding or composting takes place on this site. 3. The site is only open for the city of Winston-Salem trucks to deliver waste to the site. 4. The city contracts with Wallace Farms, Inc. to manage the operations at the facility. 5. City trucks unload leaves at the site where they are then pushed into piles. Wallace Farms personnel then manage the leaf stockpiles during their temporary storage on-site. 6. Wallace Farms transports the leaves to Wallace Farms compost facility in Davie County (permit #3004) or to the Forum 52 large Type 1 Compost Facility (permit #34-23) in Rural Hall. Wallace Farms personnel on-site stated that all leaves transported so far this year have been taken to the Forum 52 large Type 1 Compost facility for composting.
7. Wallace Farms use a ramp, stored at the facility when not in use, to load the leaves into trucks for transport as needed. 8. At the time of the inspection a small stockpile of leaves was observed onsite. 9. The facility is secured by a perimeter chain link fence and two locking gates. 10. A white dashed line marks the edge of the required buffer at the site, and all waste was well within the permitted operations area.
11. The entire site is paved with asphalt, with four small concrete pads surrounding the operations area. Gravel and rock had been spread to fill the potholes and low areas observed in the previous inspection before operations had stared.
12. The facility has a sign posted at the entrance on the gate. 13. The surface of the site is sloped so that surface water is directed away from the leaf piles. No ponding water was observed at the time of the inspection.
