Permit to Operate was issued on October 14, for 4204-CDLF-1998, Halifax County CDLF Area 1, Unit on Top of Closed
MSW Landfill, 4204-CDLF-2013 Halifax County CDLF Area 2 Phase 1 and 4204-TRANSFER-2013, Halifax County
Transfer Station. 4202-CDLF-2013 Halifax County CDLF Area 2 and 4204-Transfer-2013 were granted Life of Site
permits.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
1. The facility is fenced and is secured by a locking gate when it is not open for operation.
2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact
information as well as materials accepted at the facility.
3. The permit, operations plan, cover logs, and waste screenings were reviewed during this inspection.
4. The facility has certified personnel on site during the hours the site is open to the public. The certified personnel on
site are:

The access around the facility is stable and no soil erosion was noted.

6. No overgrown or woody vegetation was observed on the side slopes. Mr. Stanfield stated he has placed some mulch
on the side slopes to help stabilize the slopes while grass is being established.
7. Mr. Stanfield brought to the attention of Section staff that he was having an issue with a slope drain located near
the southeastern corner of the coal ash landfill. During rain events, a "rooster tail" of water would come from the
southeastern slope drain and erode the drainage ditch or roadway, depending on the intensity of the "rooster tail."
Mr. Stanfield provided videos of the phenomenon to the Section after the inspection. The southeastern slope drain
was inspected, and it appears that the water exiting the slope drain hits the concrete energy dissipater then will shoot
up into the air. The distance the water will travel from the dissipater will change depending on the intensity of the
rain fall. Mr. Stanfield asked if the slope drains or energy dissipater could be modified so that the "rooster tail" of
water does not continue to erode the side slope. Ms. Thompson forwarded the videos and photographs to Ms.
Elizabeth Werner, DWM Hydrogeologist.

8. The condition of the tipping floor is consistent with the daily operation of the facility. All waste is removed from
the tipping floor at the operating day. The tipping floor is also washed at the end of the operating day.
9. Areas of worn concrete were identified near the edge of the tipping floor and transfer trailer loading bay. Continue
to monitor and repair the tipping floor as needed as described in the Transfer Station Assessment Report.
10. Several areas of the metal push walls have been damaged by the equipment. Ensure that the push walls are monitored
and repaired as needed as described in the Transfer Station Assessment Report.
11. The transfer station walls have recently been power washed.
12. Windblown waste is being well -managed and minimal around the transfer station. Waste was identified within the
transfer trailer bay. Mr. Stanfield stated that the waste is removed from the transfer trailer bay between trailers and
at the end of the day. Continue to ensure that the windblown waste is picked up daily and removed from the transfer
trailer bay as needed throughout the operating day.
13. Several pieces of waste were observed on the grate of the leachate drain; however, the leachate drain was free
flowing.
14. The white goods were stored upright and marked after the CFCs were removed.

15. Electronic goods are stored within a building, adjacent to the white goods and metal recycling area. The electronics
have been palletized and picked up since the last inspection. Continue to ensure that the electronic goods are stored
upright to minimize glass breakage.
16. The metal recycling pile was small and well managed during this inspection.
17. Plastic pesticide containers were identified to be within the pesticide container storage area
18. Yard waste had been processed and windrowed since the last inspection.

19. The side slopes of the closed MSWLF and CDLF-1998 Area 1 were properly mowed and maintained. Good job.
20. Mr. Stanfield stated that the side slopes were monitored weekly for any evidence of leachate seeps. Mr. Stanfield
repairs any seeps as they are identified.

21. The sides slopes of the CDLF Area 2 had been properly sloped and covered with mulch since the last inspection.
Grass was beginning to grow along the side slopes.
22. Digital pictures were taken during the inspection.
