PURPOSE OF SITE VISIT: Inspection of new trucks being added to permit. STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: At the time of the 7/5/2022 inspection, firm name lettering displayed on truck # 605 did not meet height requirements in violation of the following rule. 15A NCAC 13B .0835 (b) All permitted septage management firms shall display lettering on each side of every pumper vehicle operated by the firm. The lettering shall include the firm name, town name, phone number, and septage
management firm permit number as shown on the firm application. All lettering required by this Rule on the pumper vehicle shall be no less than three inches in height and legible, distinguishable from the background, and not obstructed
from view. Identification shall not be removable (i.e. no magnetic signs).

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

(1) All required lettering indicating firm name, town name, phone number, and septage management firm
permit numbers on truck 602 was permanently attached to each side of the truck, was plainly visible and
met height requirements.
(2) All required lettering indicating town name, phone number, and septage management firm
permit numbers on truck 650 was permanently attached to each side of the truck and was plainly visible
and met height requirements. The required firm name lettering was permanently attached was plainly
visible but did not meet height requirements. All required lettering must meet rule 15A NCAC 13B .0835
(b).
(3) The tanks of both vehicles were constructed of metal and appeared to be structurally sound.
(4) The tanks of both vehicles were affixed to the truck frame with bolts.
(5) All septage drain valves appeared to be in working order. No septage leaks were observed.
(6) All port covers and caps where in place and appeared to be in proper working order.
