None.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The facility is permitted to receive, consolidate, and transfer yard trash. The site receives and manages leaves
collected curbside by the city of Winston-Salem Sanitation Division. The site receives and stores the leaves through the months of November through March. 2. No leaf grinding or composting takes place at the facility site. 3. The city contracts with Wallace Farms, Inc. to manage operations at the facility. Leaf waste is transported to Wallace Farms compost facility in Davie County (permit #3004) or to the Forum 52 large Type 1 Compost facility (permit # 34-23) in Rural Hall, Forsyth County, for processing and composting. 4. The facility annual report for the FY 2021-22 indicated that a total of 10,531.87 tons of leaf waste were collected at the site. 5. The facility is secured by a perimeter chain link fence and unauthorized access is prevented by two locking gates. 6. The site appeared to be in need of maintenance. Kudzu grown over much of the street side facing fencing and
was starting to encroach on the operating area. 7. The whole site is paved with asphalt, with four small concrete pads surrounding the operations area. 8. Several potholes and ruts were observed in the operational area. Areas of standing water with vegetative growth were also observed in the operational area. Please ensure that the potholes and low spots in the operational area are filled in and leveled prior to commencing operations to remove and prevent ponding water in and around the leaf stockpiles.
9. A white dashed line marks the 50’ buffer requirement at the site. 10. No operations were taking place during the inspection and no facility staff were onsite.
