("ATD") submits this application for a collection site permit at the following location: American Tire Distributors — Wilson 2820 Commerce Road, Wilson, Wilson County, NC 27893 ("Proposed Facility") ATD Company Contact: Jarrett Robinson Sr. Director, Safety and Business Support American Tire Distributors, Inc. 12200 Herbert Wayne Court, Huntersville, NC 28078 980-288-8181 Property Owner: David Turnage GAT Real Estate, LLC 3310 Wetherly Drive, Wilson, NC 27893 252-289-5608 ATD is a wholesaler and distributor of automotive supplies. The Proposed Facility primarily serves as a warehouse location for the aforementioned automotive supplies. Once fully operational, we expect to receive approximately 134 tons of scrap tires per month, which we will collect solely from our retail customers in and around Wilson, North Carolina. Scrap tires will not be collected or accepted from the general public. We expect to ship approximately 134 tons of tires off -site per month for recycling. On -site storage will be temporary until picked up by the recycler, or until we transport to the recycler, and is estimated to be 40 tons at one time. Initially, we will transport used automotive tires to the Wilson County Transfer Site for recycling, processing or disposal. It is our intent to eventually send the used tires directly to the recycler. Once an DocuSign Envelope ID: B5AOE31A-E118-4A90-BDB4-B662BE912B84 alternate recycling, processing or disposal facility (or facilities) is selected, the scrap tires will be transported to the recycling, processing or disposal facility by one or a combination of the following: (i) we will transport the scrap tires from the Proposed Facility to the recycling, processing or disposal facility using our fleet resources; and/or (ii) the recycling, processing, or disposal services. We are currently in discussions with local recyclers. We plan to commence operations upon permit approval. Only whole tires will be accepted into or transported from our Proposed Facility. Whole scrap tires will be recycled, processed or disposed of by local recyclers. General waste generated in connection with the collection and storage of scrap tires at the Proposed Facility will be properly disposed of at permitted transfer stations or waste collection service providers. The Proposed Facility will accumulate all scrap tires in trailers/containers for recycle; expecting 2-3 such trailers/containers on site at a time. Per 15A NCAC 13B .1105 (d) Trailers and covered roll -off containers used for the collection of scrap tires are exempt from the requirements of Rule .1106(c)(1), (c)(2), (c)(4), and (c)(6) of this Section. The Proposed Facility is served by an access road that will be kept passable at all times for any motor vehicles, including but not limited to first responder vehicles. The trailers/containers used for scrap tire storage will be locked and secured to prevent unauthorized entry or removal (e.g., glad hand and/or king pin locks). ATD will submit to the Division an annual report by August 1 of each year, for the previous calendar year, as required by section .1107(1). The report will include: • the facility name, address, and permit number; • the year covered by the report; • the total quantity and type of scrap tires received at the facility during the year covered by the report; • the total quantity and type of scrap tires shipped from the facility during the year covered by the report; • the quantity of scrap tires shipped to each receiving facility identified by name and address; and • the total quantity and type of scrap tires located at the facility on January 1 of the year covered by the report. While scrap tires will be stored in covered containers/trailers, protecting them from the environmental elements, the Proposed Facility will comply with the following technical and operational standards: • No open flames, blow torches will be allowed, nor flammable substances stored, within 50 feet of the scrap tire trailers/containers. • ATD will ensure a fire safety survey is conducted annually by local fire protection authorities. • ATD will maintain communication equipment on site to contact local fire protection authorities in case of a fire. • ATD will ensure the area within 10 feet of the scrap tire trailers/containers is free of accumulation of debris, grass, underbrush, and other potentially flammable vegetation. The manual will be updated at least once a year, or upon changes in operations at the site. The manual will include at minimum: o a list of names and numbers of persons to be contacted in the event of a fire, flood, or other emergency; o a list of the emergency response equipment at the scrap tire collection site, its location, and how it should be used in the event of a fire or other emergency; o a description of the procedures to be followed in the event of a fire, including procedures to contain and dispose of the oily material generated by the combustion of large numbers of tires; and o a listing of all hazardous materials stored on -site, their location, and information regarding precautions. In the event of a fire or other emergency that has the potential for off -site effects ATD will: o Notify the Division within 24 hours of the event. o Within two weeks of any emergency involving potential off -site impact, submit to the Division a written report describing the cause(s) of the emergency, actions taken to deal with the emergency, results of the actions taken, and an analysis of the success or failure of these actions. ATD will maintain the following records and make them available for inspection by the Division at the Division's request: o a copy of the scrap tire collection site permit; o records of the quantity of scrap tires and processed tires received at the site, stored at the site and shipped from the site, including destination (name and address of facility); and o all certification forms applicable to any tires received, stored, or shipped from the site. Scrap tires will be limited to storage in trailers, as outlined above; the anticipated cost to close scrap tire operations is approximately $8100 (for removal of three fully loaded scrap tire trailers; 150% of the anticipated maximum quantity of tires to be stored on site at any given time). Per your email correspondence with Jarrett Robinson, once the closure cost estimate has been approved, ATD will finalize the financial assurance mechanism for the permit (in compliance with Rule .1805).
