Summary: Active VIMS operating during site visit. Since previous Site Inspection, labeling
has been added to the exterior VIMS exhaust stacks, and the low vacuum alarms have
been moved to the interior of the building as recommended (Photos 5 and 6). Potential
air leakage from vicinity of suction point SSD-12 was investigated, and air within the
building is being pulled beneath interior wall adjacent to the suction point, where a
preferential pathway through the concrete slab must exist as verified using a smoke pen
(Photos 7 and 8).

V. PHOTOGRAPHS
Photo 1: Connecting Summa cannister fitting/tubing within helium shroud prior to sampling.
Sub-slab vapor-pin VMP-4 visible within cut-out in the bottom of shroud.
The site inspection was performed to observe a sub-slab sampling event being performed by AECOM
at the property. DEQ personnel arrived on-site in time to observe the first sub-slab sample collection
being performed at VMP-4 (Photos 1 through 3). Initially, there was an issue with the helium leak
test, with AECOM determining there was an air leak at the 3-way valve provided by the rental
company. Additional plastic valves were purchased, and the interior of the helium shroud was
replumbed. Following this modification, no additional issues were encountered during purge air
helium leak tests, and sub-slab sampling was able to be performed as planned.
DEQ personnel confirmed there was air leakage taking place below an interior wall adjacent to SSD-
12 while the sampling equipment was being set-up and helium tests were being performed at the
second sub-slab sampling point VMP-2R (Photo 4). This leak testing was described previously in the
Vapor Mitigation Systems Section. DEQ personnel left the property as AECOM finished collecting
sub-slab sample VMP-2R following an update with the Brownfields PM.

lamps clearly visible indoors, although audible portion of alarm is turned off according to
AECOM during prior Site Inspection
