Life of Site, issued on December 10, 2019 and expires on October 08, 2053 PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection & Resolution of the July 12, 2021 NOV STATUS OF PAST NOTED VIOLATIONS: Resolved OBSERVED VIOLATIONS: No new violations observed The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 5 ADDITIONAL COMMENTS: 01. This inspection was conducted to evaluate if the remaining cover issues were resolved from the Notice of Violation that was issued on July 12, 2021. Also a comprehensive facility compliance inspection to be conducted. 02. This facility was issued a Resolution Letter for the prior cover issues on June 10, 2022. 03. It was noted that the sedimentation pond on the west side still needs to be dredged out as soon as possible before it fills back up with water. This was discussed with Republic Staff on-site. 04. It was also noted and discussed that there were several erosion areas under the down slope drain tiles that were in need of repair. 05. It is still recommended that equipment and vehicles stay off the side slopes when it is wet due to tracking and possible exposed waste. Be sure any impacted areas are covered and seeded. 06. Facility currently only has 2 Landfill Operations Specialist (LOS): Mitch Hoggard, expires 11/15/2022 and Michael Bazemore, expires 11/15/2022. 07. Swanna (Scott Bost) has scheduled a LOP class for July 13, 2022 to certify more LOS staff. 08. Edge of Waste markers are present. 09. Screening records were available and reviewed. 10. Permit to Operate (issued 6/15/2021) and Operations Plan were available and reviewed. 11. Facility had adequate signage. 12. Facility has Locking gates and fencing for security. 13. Monitoring well viewed was locked and tagged. 14. Leachate tanks had adequate storage looking at gauges on the sides of the tanks. Leachate removal records were not reviewed. 15. Gas monitoring reports were reviewed. 16. Surface Water Monitoring reports were reviewed. 17. Interior roads were in good shape but the east side road needs to be graded. 18. Facility has added gravel to the working face and is constructing a well needed turn around area. 19. The working face is being kept smaller than it was. Cover looks better. 20. Facility staff had found and contained a new leachate seep inside of the edge of waste on the West side of the new cell being constructed. Remember that all water that comes in contact with leachate must be handled as all leachate. 21. Be sure to correct any areas were webbing was added over top of exposed. Cover and seed. 22. John College, DWM will evaluate any above items listed needing attention on July 13, 2022 while presenting at the LOS training. 23. Digital pictures were taken during this inspection with some attached to support the correction action taken to resolve the NOV and issues related to this inspection.
