PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: NA
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Bryan Leggett and David Powell rode part of the SE side of the Closed MSW along YWN area, around south
side, to north and across the top. No settled areas were visible, and site had good grass cover.
2. Around north side there was a small tree and bug shrub growing. Please, be sure and keep perimeter
vegetation under control and protect the progress that was made last year. Also, be sure to maintain access roadway around perimeter.

3. EOW markers were visible on YWN side of site and north side.
4. Composted YW can be used to reclaim slopes of Closed MSW and help with erosion control. Max depth of 2 to 3 inches should be used. Cannot be used as fill or buried etc. This should assist the County by utilizing it for a beneficial purpose. 5. Ming Chaos’ contact number is (919) 707 – 8251 and ming.chao@ncdenr.gov. Please be sure and confirm with Section Permitting before changing any procedures with the monitoring or other aspects of compliance with the Closed MSW.
