STATUS OF PERMIT:
Active – Issued August 09, 2016. Expires August 09, 2021. Pursuant to 15A NCAC 13B .0201(g), no later than
April 27, 2021, the owner or operator must submit a request to the Section for permit review and must submit updated facility plans meeting the requirements of Rule 0.565 and .0566. Packet sent to DWM. Additional information has been requested from DWM and has been sent to DWM.

ADDITIONAL COMMENTS:
01 – Facility has signage. Need to update with all required information below. Has all but the red highlighted info. 15A NCAC 13B .0566 OPERATIONAL REQUIREMENTS FOR LCIDLFS. (Rule attached to the email.) (14) A sign shall be posted at the site entrance showing the site contact's name and phone number, the permit number, emergency contact information, and the waste types accepted for disposal at the site. Change contact name. (Brent to Alicia)? 02 – Locking gates and fencing are used for security. 03 – Roads are constructed of all-weather materials and maintained well. 04 – Edge of waste markers present. Need to move inert debris back inside of the landfill edge of waste. 05 – Cover logs were reviewed during this inspection. Most of the LCID has been covered recently. Need to update
cover logs. 06 – Adequate amount of earthen cover material on-site and ready for use. 07 – must meet cover requirements that ALL waste must be covered within 30 days.
08 – NOTED DURING LAST INSPECTION: The East side slopes are in need of maintenance. The slope is 1:1.5 which has caused sever erosion during some earlier heavy rainfall events. These areas need to be
repaired/graded. This inspection: The erosion issues have been corrected but 1 small area on the north slope.
09 – Major work has been done on the North and East side slopes to obtain 3:1 side slopes. Site is starting to look really good. Continue efforts to work on side slopes. 10 – Non-Conforming waste was not an issue during this inspection.
11 – Noted During last inspection: Need to remove (ASAP) sapling trees that are growing in the landfill
permitted area before they get to 4 inches in diameter. If they reach 4 inches in diameter (like a couple), the root
ball must be removed. This inspection: Only 1 tree larger than 4 inches left to be removed. Some small
saplings still to be removed that are 2 inches or less. 12 – Incoming load receipts were not reviewed during this inspection. Please update records and have available for the next inspection. 13 – This permitted facility is not open to the public. May want to add to the sign also, but not required. 14 – Digital pictures were taken during this inspection with several attached to this report.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
