The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS 1. The Septage Land Application Site consists of Field 1 (2.5 acres), Field 2 (9.7 acres), Field 3 (4.2 acres) and Field 4 (5.7 acres), Field 5 (5.0 acres), and Field 6 (5.0) acres for a total of 32.1 acres. Field 4 is inactive at this time until trees are removed and approval by the Solid Waste Section is given for use. Total usable
acreage is 26.4 acres. 2. The site is permitted for land application of domestic septage (including portable toilet waste) and grease trap
pumpings. 3. The site is located at the intersection of Highway 18 and Glade Valley Road and has accessible all-weather
roads to the septage land application fields covered under the permit. 4. Appropriate signage was displayed at the entrances to all fields.
5. Field 1 appeared to be in good condition. Remember that all land application must be within the boundary markers. Field boundary markers on the western border was still covered with vegetation which needs to be cleared away.

7. Field 3 was observed in fair condition. All boundary markers were in place. Land application is not allowed in areas with less than 95% fescue.

8. Field 4 is inactive. No land application may occur until the trees have been removed and the Division has determined that this field is in compliance with all permit conditions and the nutrient management plan (NMP). 9. Fields 5 and 6 were observed in good condition. All boundary markers were in place. Remember, these fields need to be managed as separate fields and are not connected. Land application between the fields are outside of the field boundaries and can result in a violation. No land application can be made while baled hay is still on the field.
10. No objectionable odors were observed.
