Life of Site, issued on December 10, 2019 and expires on October 08, 2053 PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection & Pre-Construction Meeting (cell 15A) STATUS OF PAST NOTED VIOLATIONS: Resolved leachate issues, storm water perimeter ditch cleaned out and on rills and cover issues on side slopes. Some cover issues still in need of Corrective Action along the haul road and on top of landfill. OBSERVED VIOLATIONS: No new violations observed. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 7 ADDITIONAL COMMENTS: 01. This was a follow up compliance inspection to see if issues from several past inspections and the Notice of Violation have been resolved. Also, a Pre-Construction Meeting was held with Sherri Stanley and Diana Felix from the DWM Permitting Section and Christine Ritter, DWM Hydrogeologist. 02. Due to high winds (est. +/- 40kts), windblown trash was heavy. 8 to 10 staff had been deployed before this inspection to try and keep up with this issue. 03. The work that has been done on the side slopes looks great. Matting and seeding has been done. Some grass is emerging. 04. Facility has added down slope drain tiles to help direct and drain surface water off to prevent sever erosion. 05. The storm water perimeter ditch looks very good. Some minor grading still is being done. 06. It was noted that the sedimentation pond on the west side still needs to be dredged out as soon as possible before it fills back up with water. This was discussed with Wes Markussen on-site. 07. The working face is being kept smaller than it was. 08. One compactor was down for hydraulic line replacement. 09. There was exposed waste noted East of the working face and another area South of the working face that needs to be addressed within 30 days of receipt of this inspection report. These issues were discussed with Wes Markussen on-site. 10. Facility needs to keep more cover material stockpiled at the working face to achieve compliance on covering and for assisting with the reduction of windblown trash. 11. Facility staff had found and contained a new leachate seep. 12. Additional sump pumps have been added. 13. Corrective action is required due to the issues noted in this inspection report. 14. Digital pictures were taken during this inspection with some attached to support the correction action required.
