Active Life of Site Permit (PTO) 3301-CDLF-1997 issued on July 14, 2020, and expires December 30, 2057 and combined with New (PTC & PTO) 3301-COMPOST-2020 issued July 14, 2020 and expires July 14, 2030. PURPOSE OF SITE VISIT: Follow-up Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: Corrective Action in Process. OBSERVED VIOLATIONS: No new observed violations were noted during this inspection.

ADDITIONAL COMMENTS: 01 – This meeting was scheduled to discuss the Leachate Breakouts at this facility. 02 – Several new leachate breakout areas were noted. All (Old & New) were contained inside of the Edge of Waste.
03 – Staff was told to keep heavy equipment off these areas until repairs can be done. 04 – Ben Jackson (NCDEQ, DWM, Permitting Engineer) had sent John College an email discussing types of repair material (clay) and compaction.
05 – Copies of this email were handed out and discussed during this meeting. 06 – Sam Watts, S&ME will be generating a PLAN OF ACTION for these leachate breakout repairs. 07 – Copies of this Plan should be sent to ALL parties that were in attendance. 08 – Please send a copy to Ben Jackson (NCDEQ, DWM, Permitting Engineer) at: [e-mail] 09 – Another Follow-up Meeting may be scheduled when ALL parties have reviewed the Plan.
