PURPOSE OF SITE VISIT: Follow-up Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: Corrective Action in Process. OBSERVED VIOLATIONS: No new observed violations were noted during this inspection. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS: 01 – This meeting was scheduled to discuss the Leachate Breakouts at this facility. 02 – Several new leachate breakout areas were noted. All (Old & New) were contained inside of the Edge of Waste. 03 – Staff was told to keep heavy equipment off these areas until repairs can be done. 04 – Ben Jackson (NCDEQ, DWM, Permitting Engineer) had sent John College an email discussing types of repair material (clay) and compaction. 05 – Copies of this email were handed out and discussed during this meeting. 06 – Sam Watts, S&ME will be generating a PLAN OF ACTION for these leachate breakout repairs. 07 – Copies of this Plan should be sent to ALL parties that were in attendance. 08 – Please send a copy to Ben Jackson (NCDEQ, DWM, Permitting Engineer) at: benjamin.jackson@ncdenr.gov 09 – Another Follow-up Meeting may be scheduled when ALL parties have reviewed the Plan. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
