PURPOSE OF SITE VISIT: Comprehensive Follow Up Inspection – CLOSURE of YWN STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: N/A
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Section staff visited the site to confirm waste has been removed as requested and to close the Yard Waste
Notification at request of landowner Mr. Conard. NLCID N285 has already been closed and documents were recently sent to Mr. Conard for his records.
2. Site has removed the waste as requested. Some was left to reclaim slope and erosion control but a very minimal thin layer. Grass has also been established in this area. Good job. Weight tickets for documenting proper disposal have been requested multiple times and should be submitted ASAP to Mr. Powell via email, david.powell@ncdenr.gov.
View is of slope which had most of waste pushed along and dumped over hill edge. Waste has been removed and area grassed.

3. The Solid Waste Section will close this Yard Waste Notification in our system. This site will no longer be able to accept waste for staging, processing or composting. To do so may be considered illegal dumping and be in violation of Solid Waste Rules.
