PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. All photos were taken by Lee Hill on February 8, 2022. 2. The property the landfill is located on is owned by Davidson River Village, LLC and is maintained by P.H. Glatfelter, Inc. 3. The facility received 4.36 inches of rain from February 1-4, 2022. 4. The fencing for the facility along Davidson River Road was damaged during the tropical storm event that occurred August 2021. Repair the fencing to prevent unauthorized access. 5. The facility roads were constructed of gravel and were in good condition at the time of the inspection. 6. The landfill has a bentonite liner installed on the top of the portion of the cap to help reduce water infiltration into the landfill but does not extend down the side slopes of the landfill. 7. The landfill had good vegetative cover, positive drainage across the cap and no ponding water was observed. 8. Permanent edge of waste (EOW) markers constructed of a metal sign and stake are installed around the landfill and were visible at the time of inspection. 9. Groundwater monitoring wells were observed locked and appeared to be in good condition at the time of inspection. Well 6D was inundated with surface water during the tropical storm event that occurred in August 2021, but the well was not damaged. 10. A groundwater and surface water collection system is installed that uses a slurry wall and pumping system. Leachate, groundwater and storm water are pumped into an Aeration Settlement Basin (ASB) for treatment. Discharge from the basin is regulated by NCDEQ, Division of Water Resources. 11. Nineteen slope pins are installed in the surface of the landfill to monitor for possible slope movement. During the May 2019 geotechnical monitoring event four pins were not located in the same location as previous data. Weekly monitoring was conducted and the data indicated that little, if any, movement has occurred since mid - May 2019.
