Comments on COA Documentation for Madison County i My co eats on the above submittal are as follows: 1. ecord drawings for the facility should be submitted showing the following: A. Finished lines and grades of the excavated cell, liner system, and final cover; As -built location of all features of the landfill, including the leachate collection system, stormwater segregation and removal devices, and details as necessary; Locations of all in -place permeability tests, especially the 1.4x10-7 cm/s result; d. Panel layout of synthetic liner showing location of all seams and destructive samples; (Note: The topographic survey that was provided does not show all of the above and deletes the entire southwest corner of the landfill.) ►Q 3. k summary of density test results should be submitted for the soil liner and structural till areas. ith regard to the synthetic liner documentation: (See Attala's separate report; ection 2.) 4. Locations of tests aren't clear from field notes. b. Several panels appear to be drawn incorrectly on the field notes.(e.g. panels 8 & 6, panels 16 & 15, and panels 17 & 16.) Panel Ws not given for the last two pages in AttaWs report, Section 2. d. Apparently missing seam tests in the leachate pond, between panels 35 and 71, between panels 37 and 49, 71 and 72. It appears that only one destructive test was made in the pon.d.(Specs call for one every 500 linear feet.) The type of weld and test results for panel 39 are missing. g. Sheet 57-What was seamed to what? h. With regard to the extrusion weld around the bottom -It is unclear where the destructive test is. 1. No information on testing for the following seams: 67-64, 63-68, 62-68, 37-41,, 41-40, 40-39, 36-3, 72-35, 73-72, 73-81,-80,-79,-78,56- 55, 46-47, 51-52, 55-57, 75-76, 74-75, 74-58B,58-76,-75, -74, -58-B. Need copy of DEM's permit b_ 7.5 feet of head on the leachate pond may be excessive. C. Perhaps the operational plan should be more specific than to state that "leachate will be removed ... periodically..." 5. Monitoring wells on the topographic map are not numbered consistently with the original application, and one is missing entirely. 6. Buffer infringement due to flattening of north and east slopes, This needs to be addressed more thoroughly. No details were provided on the berm that was mentioned in Section 3.2.1 of the CQA Report, and it was not located on the surveyor's drawing. 7. Liner material substitution described in Section 3.4.1: Reasons given for this substitution are unsatisfactory. No slope stability calculations were used to show equivalence. 8. Final cover requirements appear to be inadequate. If the synthetic is HDPE, it must be 60 mails thick. 9. There is no general statement (with an Engineer's seal) that the facility was constructed according to the plazas and specs. 10. No photographs or slides were provided.
