ADDITIONAL COMMENTS:
01 - Signs posted at the entrance have all required information as stated in the permit and this facility is adequately
secured by fencing and locking gates.
02 - Interior roadway is constructed of all-weather material and maintained in good condition.
03 - Mulch from this facility is used for 50/50 SM cover materials for the onsite Permitted 7002-CDLF.
04 - Keep fire lanes open around of the vegetative pile at ALL times to provide access for emergency vehicles.
05 - Edge of waste markers are present.
06 - Prior information before 2020 on this Notified LCID has been recorded on inspection reports for 7002-
CDLF.
07 - The LCID area is starting to fill up. Grinding contractor needs to be notified to come to the site as soon as possible.
08 - The inert debris are being stored and to be ground for use around the roads at the Permitted 7002-CDLF.
09 - Some non-conforming waste to be pulled out of the vegetative pile before grinding.
10 - The vegetative waste has been ground into mulch since the last inspection.
11 - Overall management of this notified site is good.
12 - Digital pictures were taken during this inspection.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
