Box 849 Nashville, NC 27856 PARTICIPANTS: Amanda Thompson, NCDEQ Solid Waste STATUS OF PERMIT: 6403-LCID-2020: Permit to Construct and Permit to Operate was issued July 13, 2020, and the Permit to Operate will expire on July 13, 2025. PURPOSE OF SITE VISIT: Compliance Inspection. STATUS OF PAST VIOLATIONS None. OBSERVED VIOLATIONS None. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The all-weather roadway had been completed around the LCID. Good job! 2. Additional signage has been posted at the beginning of the roadway to the LCID, near the CDLF. 3. The working face was small. A portion of the LCID had been covered with soil. 4. The waste stream appeared to be relatively clean. A few plastic bottles were observed within the waste stream. Based on conversations with Ben Barnes, an attendant from the CDLF would monitor the LCID when loads were dumped and would remove any unacceptable wastes from the waste stream. Ensure that unacceptable wastes are removed and properly disposed of prior to covering the waste. 5. The vegetation that had previously punctured vehicle tires has been removed. It is recommended that the vegetation within the LCID be mown on a regular basis to prohibit the growth of the vegetation strong enough to puncture vehicle tires. 6. A stockpile of cover soil was observed to the south of the LCID. Several smaller piles of soil appeared to be from ditch cleaning activities and were comingled with unacceptable wastes. Ensure that the unacceptable wastes are removed prior to the utilization of the soil as cover on the LCID. 7. The sediment basin to the northeast of Phase I of the LCID has been constructed. Three porous fabric baffles run the length of the sediment basin. The sediment basin also consists of a skimmer, concrete riser, and an emergency spillway. Vegetation has begun to grow on the side slopes of the sediment basin. During this inspection, water was observed in the basin and ditch and slowly exiting the basin. Measures to monitor and maintain the basin should be put into place and followed. 8. Digital photographs were taken during the inspection.
