STATUS OF PERMIT:
4302-LCID-1998 Permit to Operate issued May 26, 2015. Permit expired on May 26, 2020. Permit renewal paperwork
was submitted May 22, 2020.
Life of Site Permits issued for 4302-CDLF-1998 and 4302-CDLF-2018 on July 23, 2020. The Life of Site Permits will
expire on March 2, 2058.
PURPOSE OF SITE VISIT:

STATUS OF PAST NOTED VIOLATIONS:

materials, structures or devices must be utilized to prevent excessive on -site erosion of the C&DLF facility or unit "
No erosion rills were identified during this inspection. Continue to repair and reseed the side slopes to prevent on -site
erosion.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
1. The facility is fenced and is secured by a locking gate when it is not open for operation.
2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact
information as well as materials accepted at the facility.
3. The facility accepts waste Monday through Saturday from 7:30 am until 4:30 pm, with the exception of Wednesday
7:30 am until 12:00 pm.
4. Five certifications were found to be expired during this inspection. The expired certifications need to be renewed
as soon as possible. Mr. Holland stated that Mr. Syck is now a part-time employee and that his certifications would
not likely be renewed. The following are facility certified personnel:

5. An area of standing water was observed in the next portion of the CDLF Area 2. Mr. Holland stated that the roadway
would be cut to allow for the drainage of the area prior to placement of waste in the area. Continue to monitor and
drain the area as needed so that waste is not disposed of in water.
6. The working face was approaching the '/z acre size limit. Six (6) inches of earthen material should be properly
applied once weekly or when the working face reaches '/z acre. The cover log was verified for September 14, 2021
through December 21, 2021. Based on the cover log, it appears that there were several gaps longer than a week
between the application of cover. Several areas of exposed waste were observed along the side slopes of the southern
portion of the CDLF Area 2. Ensure that six inches of cover is applied once weekly or when the working face
reaches 1/Z acre in size, whichever occurs first.
7. The LCID was identified to be partially covered. Ensure that the LCID is covered with six inches of soil monthly
or when the working face reaches one acre in size, whichever occurs first.
8. The waste type was reviewed at the CDLF and LCIDLF at the time of this inspection. No unacceptable waste
streams were noted in the CDLF. A pile of broken and painted pallets was observed on top of the LCID. Mr. Holland
stated that someone had mistakenly dumped the waste there and once equipment and personnel were available, the
broken pallets would be removed and properly disposed of.
9. The side slopes of the inactive CDLF Area 1 need to be mowed. The overgrown vegetation at the facility should
be mowed as often as necessary to allow for proper compliance inspection of these areas by site and Section staff.
Woody vegetation was also observed along the southern side slope of CDLF Area 1.

10. Windblown waste was observed outside of the CDLF areas at the time of this inspection. The windblown must be
collected and returned to the working face prior to close of business daily.
11. Edge of waste markers have been installed and are being maintained around CDLF Areas 1 and 2 and the asbestos
waste area. Ensure that the waste within the CDLF Area 2 is within the EOW boundaries.
12. Continue to monitor the landfills for erosion and repair any problem areas as they are observed.
