Following the installation of the turbine siphon vents on the rooftop VIMS risers, and the
extension of the VIMS riser that was sheltered from airflow by the elevator shaft and air
conditioner condensers, the system should be operating as designed, with no exposure to
public health identified during the site reinspection.

Photo 1: One of the two VIMS access panels not located during prior site inspection. The other
access panel was located within a tenant caged storage unit.
The purpose of the Site Inspection was to confirm that VIMS deficiencies identified during
the March 24, 2021 Site Inspection had been properly addressed. Per the previous Site
Inspection: During the inspection, the following were not complete, missing, or not located:
proper placarding, two VIMS riser access panels, and riser turbines and labeling.

During the August 3, 2021 re-inspection, it was determined that all of the issues identified
previously had been properly addressed with the exception of signage that should have been
installed within electrical/mechanical rooms as called for in the approved VIMS Plan. This
signage would identify the presence of the VIMS and warn against installing floor
penetrations without taking the proper precautions. These placards were not installed prior
to the follow-up Inspection; however, it was agreed during the inspection that if these signs
were installed relatively quickly, and photographs provided to DEQ, that it would be DEQ’s
position that all the system requirements had been addressed.

On August 10, 2021, the required signage was installed, and DEQ was notified the same day.
This notification included photos of the placards installed, and a placard photo is included as
Photo 6 within this Inspection Report, with Terracon’s notification and photos attached.
