OBSERVED VIOLATIONS:
1. 15A NCAC 13B .0203(d) states: "By receiving solid waste at a permitted facility, the permittee shall be considered
by the Department to have accepted the conditions of the permit and shall comply with the conditions of the permit. "

6(d) of Permit Approval to Operate 8004T-Transfer-1995 states: "Effective vector control measures must be
applied at all times to control any potential vectors including flies, rodents, insects, and other vermin. "

The approved Facility Operations Plan Waste Management of Carolinas, Inc. dated January 30, 2018 (DIN
28731, Page 5) "Housekeeping & Vector Control' states in part: "Wastes will only be deposited on the facility's
tipping floor, or deposited directly into a waste transfer container; and no waste will be stored on the tipping floor
after operating hours. Waste maybe stored on -site overnight as outlined above in Section 3.0 — Equipment; however,
in no case will the storage of the waste cause a nuisance, such as odor or attraction of vectors. Effective vector
control measures will be applied at all times to control for potential vectors including flies, rodents, insects, and other
vermin. "
During the inspection four rodents were observed in the trailer loading bay area.
Waste Management of the Carolinas, Inc. is in violation of 15A NCAC 13B .0203(d) in that effective vector
control measures are not being applied at all times to control any potential vectors including flies, rodents,
insects, and other vermin. To achieve compliance, Waste Management of Carolinas, Inc. must ensure effective
vector control measures are applied at all times to control any potential vectors including flies, rodents, insects,
and other vermin.
2. 15A NCAC 13B .0405(a)(4) states: "Owners or operators of a site shall operate and maintain the site to prevent on -
site populations of vectors. "
During the inspection four rodents were observed in the trailer loading bay area.
Waste Management of the Carolinas, Inc. is in violation of 15A NCAC 13B .0405(a)(4) in that the owners or
operators are not maintaining the site to prevent on -site populations of vectors. To achieve compliance, Waste
Management of Carolinas, Inc. must operate and maintain the site to prevent on -site populations of vectors.
3. 15A NCAC 13B .0203(d) states: `By receiving solid waste at a permitted facility, the permittee shall be considered
by the Department to have accepted the conditions of the permit and shall comply with the conditions of the permit. "

6(e)(ii) of Permit Approval to Operate 8004T-Transfer-1995 states: "Windblown materials must be collected by
the end of the day and no windblown material may be allowed to leave the facility boundary. "
During the inspection windblown material was observed accumulated around the truck turn around area in front of the
transfer station and along the north outside wall of the transfer station.
Waste Management of the Carolinas, Inc. is in violation of 15A NCAC 13B .0203(d) in that windblown litter
from site operations was not picked up by the end of each operating day. To achieve compliance, Waste
Management of Carolinas, Inc. must collect all windblown material by the end of each operating day.
4. 15A NCAC 13B .0405(a)(10) states: "Site staff shall conduct daily inspections for windblown waste on the
site property. Windblown litter from site operations discovered during the daily inspections or observed on
adjacent properties shall be picked up and containerized for disposal by the end of each operating day,
unless the landowner of the adjacent property denies access to site staff. The site shall prevent waste from
being blown outside the waste handling areas by the wind, using methods such as:
(A) requiring that vehicles entering and leaving the site keep waste covered;
(B) providing skirts, such as rubber belting or brushes, around the top of chutes to minimize the
space between the chute and the hauling trailer at sites with chutes and hoppers; or
(C) preventing waste from leaving the site using methods such as fencing, netting, or diking. "
During the inspection windblown material was observed around the truck turn around in front of the transfer station
and the north outside wall of the transfer station.

Waste Management of the Carolinas, Inc. is in violation of 15A NCAC 13B .0405(10) in that windblown litter
from site operations is not being picked up by the end of each operating day. To achieve compliance, Waste
Management of Carolinas, Inc. must collect all windblown material by conducting daily inspections or
implement measures to prevent waste from leaving the transfer building.

area must be maintained in a clean, sanitary condition at all times and must be cleaned at least daily. "
During the inspection the trench drain was clogged with debris and the entrance to the tipping floor had a heavy layer
of debris. Waste was observed deposited on the I beams in the trailer loading bay.

Waste Management of the Carolinas, Inc. is in violation of 15A NCAC 13B .0203(d) in that the tipping floor
and trailer loading bay is not maintained in a clean, sanitary condition at all times. To achieve compliance,
Waste Management of Carolinas, Inc. must clean and ensure the tipping floor, trench drain and trailer loading
bay are maintained in a clean, sanitary condition at all times and cleaned at least daily.
15A NCAC 13B .0405(a)(11) states: "Unless otherwise stated in the site permit, all waste shall be removed from the
tipping floor, the truck loading bays, and from behind push walls by the end of each day of operation and disposed of
in accordance with this Subchapter. The tippingfloor, push walls, and truck loading bays shall be cleaned with a
pressure washer no less than once per month. The remaining areas of the site building including side walls and any
material storage areas outside of the building shall be cleaned with a pressure washer no less than twice per year.
Wash water generated from cleaning waste handling areas shall be contained and treated as leachate. Cleaning and
maintenance records shall be maintained and made available to the Division upon written request. "

Waste Management of Carolinas, Inc. must ensure all extraneous waste is removed from the tipping floor,
truck loading bay, and from behind push walls by the end of each day.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day maybe assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
1. Waste Management of Carolinas, Inc. owns the facility, and contracts with Custom Ecology, Inc. (CEI) to operate
the transfer station.
2. The required signage is posted at the entrance of the facility.
3. The facility is secured by a gate at the entrance to prevent unauthorized access.
4. The facility accepts an average of 156 tons of waste each operating day.
5. Leachate is discharged into the Rowan County Sanitary Sewer system. The grit chamber and oil -water separator
were inspected and cleaned out by Crystal Clean on October 15, 2021.
6. Anderson Pressure Washing, Inc. was on -site October 30, 2021 to pressure wash the inside and outside of the
transfer station.
7. A roll -off is staged beside the transfer station for temporary storage of wooden pallets.
8. During the inspection, wooden pallets mixed with waste were observed on the tipping floor. An unacceptable load
consisting mainly of wooden pallets was observed being dumped during the inspection. Facility staff separated this
load from the rest of the waste for proper disposal. Following the inspection Mr. Owens provided proof this load
was disposed at Rowan County Landfill wood waste processing area (Permit Number 80-03).

9. Random inspection forms were provided via email. Records reviewed were from January 2021 to October 2021.
Ensure facility staff are documenting unacceptable waste and final disposal.
10. Certified Operator: Samuel Mufaro — Transfer Station Operations Specialist expires February 9, 2024. Mr. Mufaro
is the only certified operator at this facility. 15A NCAC 13B .0405(a)(7) states: "During hours of operation, an
operator trained in accordance with G.S. 130A-309.25 shall be on -site. Sites shall provide all staff with no less
than eight hours of training updates annually that includes a review of the operations plan andpermit
documents. Documentation of the training shall be placed in the operating record and provided to the Division
upon written request. " A certified operator must be at the facility at all times while the facility is in
operation.
