The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The required signage is posted at the entrance of the facility
2. The facility is secured by an automatic gate at the entrance to prevent unauthorized access.
3. The facility is open from 8 am to 4 pm Monday through Friday.
4. The facility is permitted for the treatment of medical waste with steam sterilization, as well as the storage and
processing of treated and untreated medical waste.
5. The facility is permitted to use one sterilization (autoclave) unit.
6. The operating floor, trailer storage area and unloading docks are cleaned daily.
7. The floor of the operational pit was clean and free of debris.
8. Floor drains are connected to the City of Gastonia sanitary sewer system. Facility staff have installed screens on the
drain grates to prevent waste from entering the floor drains and the screens are changed monthly.

9. Facility staff pick up windblown litter periodically during the day around the compactor and at the end of each day.
10. All trucks are equipped with spill kits.
11. Treated medical waste is loaded into a compactor, then transported to CMS Landfill in Concord for disposal.
12. The facility has a refrigerated trailer on -site.
13. Every Tuesday hazardous waste is transported to Heritage in Charlotte, NC for incineration.
14. All pathological waste is transported to Covanta in Atlanta, GA for incineration.
15. All medical waste received is processed by the next day.
16. All recycled containers are washed, disinfected and inspected for damage prior to being returned to customers.
17. All treatment carts are relined after each autoclave cycle.

18. Once a week the autoclave is tested to ensure the sterilization is being achieved. Spore test data records (Autoclave
Quality Control Log) were reviewed from August 6, 2020 to November 30, 2021.
19. Reviewed RMW (Regulated Medical Waste) Container Logs from April 30, 2021 to November 30, 2021.

20. Damaged sharp containers are palletized and shipped to Daniels Sharpsmart's warehouse in Kenosha, Wl for
repairs.
21. Reviewed the Autoclave Cooks Log from July 2021 to November 2021.
22. Reviewed the Washline Daily Operators Log from August 8, 2020 to November 20, 2021.
23. The facility processed 3,656,046 lbs. of medical waste from July 2020 through November 2021.
