The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected four vehicles – Tag#JV-9510, VIN# correct per the firm application, Tag#JY-9345, VIN# correct per
the firm application, Tag#JS-1013, VIN# correct per the firm application and a new vehicle Tag#HMT-3068,
VIN# 1GDG5C1G87F903965, 400 gallon capacity. The tanks appeared structurally sound & securely attached
on these four vehicles. Outlet covers were on and no drips/leaks were noted. The lettering was correct on both
sides of all four vehicles per info on the firm application. However, vehicle Tag#HMT-3068 has part of the
lettering hidden by a strap which needs to be moved.
2. Vehicle Tag#KE-7172 was inspected in December 2020 and did not need to be inspected at this time.
3. Reviewed the truck pumping logs and it appears that the required information is being maintained.
4. Mr. Rinaldi stated he had attended a continuing education class in 2021. The firm permit renewal process is
currently underway. Ensure info for all five vehicles is listed on the new firm application.
