OBSERVED VIOLATIONS:
1. 15ANCAC 13B .0835(b) All permitted septage management firms shall display lettering on each side of every
pumper vehicle operated by the firm. The lettering shall include the firm name, town name, phone number, and
septage management firm permit number as shown on the firm application. All lettering required by this Rule on
the pumper vehicle shall be no less than three inches in height and legible.
Vehicle Tag# 8922PT had part of the firm name, the town name and the phone number at less than three
inches in height on both sides of this vehicle. Vehicle Tag# P766747 had damaged decals on both sides of
this vehicle.
Correct the lettering as needed & submit photos (which include a ruler for height reference) by 12/17/2021.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle Tag#P770155, VIN#1HTWNAZT6CJ127190 – the tank appeared structurally sound &
securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of
this vehicle per info on the firm application.
2. Inspected two new vehicles that were not listed on the firm application. Tag#P766747,
VIN#1FVHGSFE2MHMP6129, 2700 gallon capacity and Tag#8922PT, VIN#1UNST42244L023905, 6300
gallon capacity. The tanks appeared structurally sound & securely attached. Outlet covers were on and no
drips/leaks were noted. Lettering issues were noted which are explained on the front page of this report.
3. The truck pumping logs were reviewed and it appears that the required information is being maintained.
However the physical location of each pump job is not clearly identified – add FT Bragg to the building
numbers listed for FT Bragg.
4. Two vehicles listed on the firm application (Tag# P760502 & Tag# PV46513) are no longer in service and were
not inspected at this time. Vehicle Tag# P760508 was inspected in October 2020 and did not need to be
inspected again at this time.
5. Upon discussion it appears that the required continuing education hours have been obtained. The firm permit
renewal process is currently underway. Ensure that the correct info for each of the four vehicles to be used in
2022 is listed on the new firm application.
