The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected two vehicles, Tag#YA65255, VIN# correct per the firm application & Tag#YA137262, VIN# correct
per the firm application. The tanks appear structurally sound & securely attached on both vehicles. The outlet
covers were on both vehicles and no drips/leaks were noted. The lettering is correct on both sides of these
vehicles per info on the firm application except that “Fayetteville” is the only town name that is listed on the
vehicles - but that town name is missing from the current firm application (Autryville is the only town name
currently listed). Mr. Autry is going to send a corrected firm application to the Raleigh Central office to also list
Fayetteville. Also the lettering is severely faded on one of the vehicles’ tank sides, but the lettering is also on
the truck doors & in compliance.
2. Reviewed the truck pumping logs and it appears that the required information is being maintained.
3. Mr. Autry stated that he had attended a continuing education class in 2021. Discussed that the firm permit
renewal process is currently underway.
