STATUS OF PAST NOTED VIOLATIONS:
Truck logs were not made available for review in 2013, 2015 or on the date of this inspection.
OBSERVED VIOLATIONS:
15A NCAC 13B .0836 (a) Each permit holder shall maintain a log that includes the following information for each
septage pumping event. (1) the date, type, quantity, and location of septge pumped; the location for tanks shall be a street
address and the location of portable toilets shall be a route; and (2) the location of the discharge of the septage. (b) A
septage management firm shall make all records, documents, or logs required in accordance with this Section or
conditions of the permit available for review by the Division at the time and place of an inspection of the firm’s septage
pumper trucks or upon the Division’s request.
The truck pumping logs were not made available for review at the time of this inspection. This violation has
previously been noted (see above). Gave Mr. Carter an example log & discussed the required information to be
maintained. Submit a few recent pages of the truck pumping logs for review by December 10th so that the firm
permit may be reissued & so that you may continue to operate in 2022.

ADDITIONAL COMMENTS
1. Inspected vehicle Tag#YA108355, VIN# correct per the firm application. The tank appears structurally sound &
securely attached (side rail is rusted). Outlet covers were on and no drips/leaks were noted. The required
lettering is painted on and is correct on both sides of this vehicle per info on the firm application.
2. Mr. Carter stated that he had attended a continuing education class this year. Discussed that the firm permit
renewal process is currently underway.

enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
