SEE BACK OF PAGE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Inspected vehicle Tag#YA142407, VIN# 1HTHC8TRXXH613574. The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of this vehicle except that the town name on the current firm application is “Shannon” but the town name on the vehicle is Lumberton. Mr. Hunt stated that this vehicle would soon be repainted – when this occurs please ensure that the required lettering has the correct info per the current firm application and that all letters & numbers are at least three inches in height. Contact the Raleigh Central office to schedule a truck inspection for this repainted truck. Mr. Hunt stated that he was working on a second vehicle which will also need an inspection. 2. The truck pumping logs were reviewed and it appears that the required information is being maintained except that the town name needs to be recorded as part of the physical location for each pump job. Also Mr. Hunt needs to list which land application site was used for any discharge event (SLAS 78-20 or SLAS 83-04). 3. Mr. Hunt stated that he had attended a continuing education class in 2021. Discussed that the firm permit renewal process is currently underway. Mr. Hunt indicated that he would be paying for 2 vehicles for 2022. Also ensure that all authorization forms are submitted to the Raleigh Central office for any disposal locations to be used in 2022 - including land app sites or storage tanks permitted to Cooper Septic.
