or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle Tag#YA145048, VIN# 1HTSDPNN1PH529871 – this is not the tag & VIN numbers listed on
the firm application. The tank appears structurally sound & securely attached. Outlet covers were on and no
drips/leaks were noted. The lettering is correct on both sides of the vehicle except that the phone number listed
is 910-486-5335, which is not the phone number listed on the firm application.
2. Discussed that the firm permit renewal process is currently underway. Ensure that the correct tag & VIN
numbers are listed on the application. Also ensure that the phone number displayed on the truck (910-486-5335)
is listed on the firm application. Mr. Carter stated that he had attended a continuing education class in 2021.
3. Reviewed the truck pumping logs. It appears that the required information is being maintained however the
town name for each job needs to be listed – begin recording the full physical address.
