Summary: Several monitoring well vaults are located near the corner of N. Elm and
English Streets (Photo 6 and 7). At least one of the well vault covers is missing, and the
well had been grouted to land surface. No attempt was made to remove the remaining
well vault covers during the Site Inspection.
No risk to the Public was observed at the time of the Site Inspection.

V. PHOTOGRAPHS
Photo 1: View of former textile mill building facing west. Future patio renovation area is
located to the left of the former smokestack.
Proposed soil grading work that is covered in an approved EMP had not begun at the
time of the Site Inspectioon. The existing concrete patio outside the future restaurant
space will be regraded and expanded into an existing parking lot which is currently at a
higher elevation.

Photo 3: A portion of higher grade that will be lowered as a part of the planned patio expansion.
Patio will be expanded past the roll-off dumpster on the right side of the photograph.
