(1) PLAN AND PERMIT REQUIREMENTS
(a) Construction plans shall be approved and followed.
— (b) Specified monitoring and reporting requirements shall be met.
(2) SPREADING AND COMPACTING REQUIREMENTS
(a) Solid waste shall be restricted into the smallest area feasible.
— (b) Solid waste shall be compacted as densely as practical into cells.
(3) COVER REQUIREMENTS
— (a) Solid waste shall be covered after each day of operation, with a
compacted layer of at least six inches of suitable cover or as
specified by the division.
(b) Areas which will not have additional wastes placed on them for
12 months or more shall be covered with a minimum of one foot
of intermediate cover.
(c) After final termination of disposal operations at the site or a
major part thereof or upon revocation of a permit, the area shall
be covered with at least two feet of suitable compacted earth.
(4) EROSION CONTROL REQUIREMENTS
— (a) Adequate erosion control measures shall be practiced to pre-
vent silt from leaving the site.
— (b) Adequate erosion control measures shall be practiced to pre-
vent excessive on -site erosion. I .
(5) DRAINAGE CONTROL REQUIREMENTS
(a) Surface water shall be diverted from the operational area.
(b) Surface water shall not be impounded over or in waste.
— (c) Completed areas shall be adequately sloped to allow surface
water runoff in a controlled manner.
(6) VEGETATION REQUIREMENTS
— (a) Within six months after final termination of disposal operations
at the site on a major part thereof or upon revocation of a
permit, the area shall be stabilized with native grasses.
(b) Temporary seeding will be utilized as necessary to stabilize the
site.
(7) WATER PROTECTION REQUIREMENTS
(a) The separation distance of four (4) feet between waste and
water table shall be maintained unless otherwise specified by
the division in the permit.
(b) Solid waste shall not be disposed of in water.
— (c) Leachate shall be contained on site or properly treated prior to
discharge. An NPDES permit may be required prior to the dis-
charge of leachate to surface waters.
(8) ACCESS AND SECURITY REQUIREMENTS
(a) The site shall be adequately secured by means of gates, chains,
berms, fences, and other security measures approved by the
division, to prevent unauthorized entry.
— (b) An attendant shall be on duty at the site at all times while it is
open for public use to ensure compliance with operational
requirements.
(c) The access road to the site shall be of all-weather construction
and maintained in good condition.
(d) Dust control measures shall be implemented where necessary.

(9) SIGN REQUIREMENTS
— (a) Signs providing information on dumping proced u res, the hours
during which the site is open for public use, the permit number
and other pertinent information shall be posted at the site
entrance.
(b) Signs shall be posted stating that no hazardous or liquid waste
can be received without written permission from the division.
(c) Traffic signs or markers shall be provided as necessary to pro-
mote an orderly traffic pattern to and from the discharge area
and to maintain efficient operating conditions.
(10) SAFETY REQUIREMENTS
— (a) Open burning of solid waste is prohibited.
— (b) Equipment shall be provided to control accidental fires or
arrangements shall be made with the local fire protection
agency to immediately provide fire -fighting services when
needed.
— (c) Fires that occur at a sanitary landfill shall be reported to the
division within 24 hours and a written notification shall be
submitted within 15 days.
— (d) The removal of solid waste from a sanitary landfill is prohibited
unless the owner/operator approves and the removal is not
performed on the working face.
(e) Barrels and drums shall not be disposed of unless they are empty
and perforated sufficiently to ensure that no liquid or hazard-
ous waste is contained therein.
(11) WASTE ACCEPTANCE AND DISPOSAL REQUIREMENTS
— (a) A site shall only accept those solid wastes which it is permitted to
receive.
(b) No hazardous, liquid, or infectious waste shall be accepted or
disposed of in a sanitary landfill except as may be approved by
the division.
(c) Spoiled foods, animal carcasses, abattoir waste, hatchery waste,
and other animal waste delivered to the disposal site shall be
covered immediately.
(d) Asbestos waste that is packaged in accordance with 40 CFR 61, as
amended through January 1,1985, may be disposed of separate
and apart from other solid wastes at the bottom of the working
face or in an area not contiguous with other disposal areas, in
either case, in virgin soil. Separate areas shall be clearly marked
so that asbestos is not exposed by future land -disturbing activi-
ties. The waste shall be covered immediately with soil in a
manner that will not cause airborne conditions. Copies of 40
CFR 61 may be obtained and inspected at the Solid and Hazard-
ous Waste Management Branch, Division of Health Services,
P. O. Box 2091, Raleigh, NC, 27602.
(12) MISCELLANEOUS REQUIREMENTS
— (a) Effective vector control measures shall be applied to control j
flies, rodents, and other insects or vermin when necessary.
(b) Appropriate methods such as fencing and diking shall be pro- I
vided within the area to confine solid waste subject to be blown
by the wind. At the conclusion of each day of operation, all
windblown material resulting from the operation shall be col-
lected and returned to the area by the owner or operator.

Re: Installm-ent of Ground -water Monitoring Wells at the Town of
Canton Landfill (Permit #44-04)
Dear: Mayor Hardin,
As referenced in the letters from this office dated March 31,
1987 and September 21, 1987, solid waste facilities are required to
construct ground -water monitoring wells and implement a ground and
surface water monitoring program by June 30, 1989. The ground -water
monitoring system should be constructed and implemented in a manner
consistent with the specifications of the "North Carolina Water
Quality Monitoring Guidance Document for Solid Waste Facilities"
(copy enclosed).
In order to comply with these requirements, you will need to
submit a ground -water monitoring plan to this office which contains
the following information:
1. A map (preferably 8 1/2" x 1111) delineating the entire
landfill and adjacent property owned (or leased) by you.
This map should clearly show the location of the landfill
with respect to the entire property.
2. A specific site map of the landfill itself
illustrating the configuration of the landfill per the
original permit description. On this map also indicate the
following:
a. Areas of past landfilling activity
b. Area of present landfilling activity

C. Area of proposed future landfilling activity
d. Areas of demolition waste landfilling
e. Proposed monitoring well locations. Each well should
be numbered on the map (e.g. MW-l; MW-2; etc...).
Note: Upon construction, these wells must also be
permanently numbered in a corresponding fashion in the
field.
In designing an acceptable ground -water monitoring system, you
should install at least one upgradient (background) well and a
sufficient number of downgradient monitoring wells at locations which
(based upon topography and known hydrogeological features) are
believed to adequately represent the impact of the landfill on the
groundwater of the area. The screened intervals shall be established
in the most conductive zones based upon field observations of the
material encountered during well boring operations.
- Proposed--locations--f-or theplanned ground -water monitoring wells
should be submitted to our office for approval prior to actual well
drilling operations. Again, it is very important that wells be
installed according to the specifications in the "North Carolina
Water. Quality Monitoring Guidance Document for Solid Waste
Facilities". Wells should be set within the permitted area for the
solid waste facility in areas free of garbage, refuse, or
sedimentation. If circumstances prevent the placing of wells inside
the property boundaries, contact this office for assistance.
Should the landfill be in operation at the time EPA Subtitle "D"
proposed regulations become effective at a future date, additional
hydrogeological information may become necessary to comply with these
regulations. other changes in State or Federal regulations, or
changes in the operation of the landfill may also result in future
changes to upgrade the ground -water monitoring system.
Within 15 days upon the completion of well installation, a
complete and accurate Well Completion Record (DHS 3342) should be
submitted to this office for all ground -water monitoring wells.
These records should clearly indicate the following:
- Well construction specifications pertaining to inner/outer
casing diameter
- Borehole depth
- 24-hour water table elevations
- Screen length
- Screen slot size
- Vertical extent of screen (e.g. where the screen is placed
in the aquifer) .

- All other data required on well construction forms as
specified in the "N.C. Water Quality Monitoring Guidance
Document for Solid Waste Facilities".
In addition, please submit to us a summary of the hydro -
geological data accumulated for the landfill site from the boring
operation. Include boring logs and other pertinent subsurface
information. Also, within 15 days of well installation, designate in
writing to this office the name of the contractor you have chosen to
perform your annual ground and surface water monitoring extractions,
and the date on which you plan to perform your annual sampling.
If your facility is already in the process of establishing a
groud-water monitoring system, please inform our office immediately
as to what stage of development you are in toward meeting the June
30, 1989 deadline. If you have further questions or comments please

Dear Mr. Lyda:
The information you recently submitted to our office (regarding the proposed
locations for ground -water monitoring wells at the Canton Landfill) was not
sufficiently detailed for us to properly assess the adequacy of your monitoring
P lExn, Please resubmit a monitoring plan within 15 days providing the information
requested in the May 5, 1989, letter from our office. I have enclosed information
on "Submitting a Ground -water Monitoring Plan" which covers basically the same
information included in the May 5th letter.
In as much as the June 30, 1989, deadline for establishing a ground -water
monitoring system has already passed, it is very important that you resubmit a
P lanimmediately and proceed with well installation as soon as the well locations
are approved by our office. Generally at least one upgradient (background) well
and a minimum of two or three downgradient wells are necessary to determine the
impact of the landfill upon the ground -water of the area. The two wells proposed
wouldprobably not be adequate to do this. Also, the monitoring wells should be
located within the permitted boundaries of the landfill facility. The proposed
wrz:� 11#2 appears to be in a location outside the landfill boundary.

'RE: Siting of Ground -water Monitoring Wells at Sanitary Landfill
Facilities
Dear Mr. Lyda:
Mike Babuin, of our office, told me that you had some questions
regarding my July 10, 1989, letter to you. The purpose of this
letter is to answer these questions relating to siting of monitoring
wells.
All monitoring wells should be located within the permitted
boundaries of the landfill facility. This policy has been
established for a couple of reasons. First, the permitted area is
the area regulated by the Solid Waste Management Section of the
Division of Health Services. Because this is the area of our
jurisdiction, our office must know the impact of the landfill on the
ground -water of the permitted area. Secondly, the proper design of a
ground -water monitoring system dictates that monitoring wells be
placed close to the source of potential contamination in order to
enable early detection and assessment of any ground -water
contamination. This policy protects you from a liability standpoint,
since early detection of ground -water contamination allows you more
time to react with the proper assessment and (if necessary)
corrective action prior to the contaminant plume moving across
permitted boundaries into an aquifer that may be a source of private
water supplies.
In establishing an adequate ground -water monitoring system, it
is important th4t there be at least one upgradient (background) well
and a sufficient number of downgradient monitoring wells "to detect
the effects of the facility on the ground -water in the area" (North
Carolina Solid Waste Management Rules, 10 NCAC 10G .0601, a).

Generally, in order to adequately determine the impact of the
landfill on ground -water downgradient of the facility, several wells
or well nests would be required. The number and location of
downgradient wells which will be needed to establish an adequate
monitoring system must be determined by the site -specific
hydrogeological characteristics at the facility. Rarely would only
one downgradient well be adequate to characterize the impact of the
landfill upon the ground -water of the permitted area. Our office

previously submitted was insufficient to properly assess the adequacy
of your proposed ground -water monitoring system. Sufficient maps and
hydrogeological data should be submitted to our office that support
the number and location of monitoring wells you propose in your
ground -water monitoring plan.
Please provide our office with the information requested as
delineated in the enclosure "Submitting a Ground -water Monitoring
Plan" sent with my last letter of July 10, 1989. A detailed small
scale topographic map of the facility would be especially helpful if
there is not a lot of hydrogeological data available for the landfill

SIR: AN INSPECTION OF YOUR LAND DISPOSAL SITE HAS BEEN MADE THIS DATE AND YOU ARE NOTIFIED OF THE VIOLATIONS, IF ANY, MARKED BELOW
WITH A CROSS (X).
(1) PLAN AND PERMIT REQUIREMENTS
— (a) Construction plans shall be approved and followed.
(b) Specified monitoring and reporting requirements shall be met.
(2) SPREADING AND COMPACTING REQUIREMENTS
(a) Solid waste shall be restricted into the smallest area feasible.
(b) Solid waste shall be compacted as densely as practical into cells.
(3) COVER REQUIREMENTS
— (a) Solid waste shall be covered after each day of operation, with a
compacted layer of at least six inches of suitable cover or as
specified by the division.
(b) Areas which will not have additional wastes placed on them for
12 months or more shall be covered with a minimum of one foot
of intermediate cover.
(c) After final termination of disposal operations at the site or a
major part thereof or upon revocation of a permit, the area shall
be covered with at least two feet of suitable compacted earth.
(4) EROSION CONTROL REQUIREMENTS
(a) Adequate erosion control measures shall be practiced to pre-
vent silt from leaving the site.
(b) Adequate erosion control measures shall be practiced to pre-
vent excessive on -site erosion.
(5) DRAINAGE CONTROL REQUIREMENTS
— (a) Surface water shall be diverted from the operational area.
(b) Surface water shall not be impounded over or in waste.
(c) Completed areas shall be adequately sloped to allow surface
water runoff in a controlled manner.
(6) VEGETATION REQUIREMENTS
(a) Within six months after final termination of disposal operations
at the site on a major part thereof or upon revocation of a
permit, the area shall be stabilized with native grasses.
(b) Temporary seeding will be utilized as necessary to stabilize the
site.
(7) WATER PROTECTION REQUIREMENTS
— (a) The separation distance of four (4) feet between waste and
water table shall be maintained unless otherwise specified by
the division in the permit.
(b) Solid waste shall not be disposed of in water.
(c) Leachate shall be contained on site or properly treated prior to
discharge. An NPDES permit may be required prior to the dis-
charge of leachate to surface waters.
(8) ACCESS AND SECURITY REQUIREMENTS
— (a) The site shall be adequately secured by means of gates, chains,
berms, fences, and other security measures approved by the
division, to prevent unauthorized entry.
— (b) An attendant shall be on duty at the site at all times while it is
open for public use to ensure compliance with operational
requirements.
— (c) The access road to the site shall be of all-weather construction
and maintained in good condition.
— (d) Dust control measures shall be implemented where necessary.

(9) SIGN REQUIREMENTS
(a) Signs providing information on dumping procedures, the hours
during which the site is open for public use, the permit number
and other pertinent information shall be posted at the site
entrance.
— (b) Signs shall be posted stating that no hazardous or liquid waste
can be received without written permission from the division.
— (c) Traffic signs or markers shall be provided as necessary to pro-
mote an orderly traffic pattern to and from the discharge area
and to maintain efficient operating conditions.
(10) SAFETY REQUIREMENTS
(a) Open burning of solid waste is prohibited.
—(b) Equipment shall be provided to control accidental fires or
arrangements shall be made with the local fire protection
agency to immediately provide fire -fighting services when
needed.
— (c) Fires that occur at a sanitary landfill shall be reported to the
division within 24 hours and a written notification shall be
submitted within 15 days.
— (d) The removal of solid waste from a sanitary landfill is prohibited
unless the owner/operator approves and the removal is not
performed on the working face.
(e) Barrels and drums shall not be disposed of un less they are empty
and perforated sufficiently to ensure that no liquid or hazard-
ous waste is contained therein.
(11) WASTE ACCEPTANCE AND DISPOSAL REQUIREMENTS
(a) A site shall only accept those solid wastes which it is permitted to
receive.
— (b) No hazardous, liquid, or infectious waste shall be accepted or
disposed of in a sanitary landfill except as may be approved by
the division.
(c) Spoiled foods, animal carcasses, abattoir waste, hatchery waste,
and other animal waste delivered to the disposal site shall be
covered immediately.
— (d) Asbestos waste that is packaged in accordance with 40 CFR 61, as
amended through January 1,1985, may be disposed of separate
and apart from other solid wastes at the bottom of the working
face or in an area not contiguous with other disposal areas, in
either case, in virgin soil. Separate areas shall be clearly marked
so that asbestos is not exposed by future land -disturbing activi-
ties. The waste shall be covered immediately with soil in a
manner that will not cause airborne conditions. Copies of 40
CFR 61 may be obtained and inspected at the Solid and Hazard-
ous Waste Management Branch, Division of Health Services,
P. O. Box 2091, Raleigh, NC, 27602.
(12) MISCELLANEOUS REQUIREMENTS
— (a) Effective vector control measures shall be applied to control
flies, rodents, and other insects or vermin when necessary.
— (b) Appropriate methods such as fencing and diking shall be pro-
vided within the area to confine solid waste subject to be blown
by the wind. At the conclusion of each day of operation, all
windblown material resulting from the operation shall be col-
lected and returned to the area by the owner or operator.
