Please note that we have notified the North Carolina Department of Environment and Natural Resources several times that
we are no longer Collins & Aikman - We are now IAC (International Automotive Components) and have been since October
of 2007 and our reports have all reflected this. How do we get them to change our records???

Dear Facility Contact Jack Davis:
Your facility is required to complete and submit an annual report for solid waste activities. You need to have
your form completed and submitted no later than August 30, 2009. The forms for this reporting can be found
on our website at:

When you visit the website you will see the links to the pdf forms. You will need Adobe Reader to open the
files. It is probably already on your computer, but if not you can download the program for free from the
Adobe website (htti)://www.adobe.com/products/acrobat/readstgp2.html).
According to our records, you need to complete the INDUS LF form. We ask that you make sure to use the
appropriate form for your facility type as each form contains facility -specific questions. Besides using the
correct facility form, you will also need to ensure that you use the following information when filling -in the top
line of the form. Where your form asks for:
Facility Name, please type COLLINS & AIKMAN
Permit, please type 5603-INDUS-1981
ID, please type P0093
Taking time to get these initial steps correct will speed up the processing of your information and
keep you from having to re -do your form.
The questions on the form are same ones that were on the form last year, but in a slightly different order. Most
of the forms include a question "Indicate type and tonnage of material separated from received waste". This
question refers to recycling or recovery of materials after the waste was received at the facility. Single stream
recycling, such as glass, cardboard and aluminum cans received in a separate area will not be addressed on this
form but should be accounted for within the Annual Local Government Solid Waste Management Reporting
Forms.
Please try to answer all the questions on your form as accurately and completely as possible. If you are unsure
what is being asked for or get stuck, there are two sources for help. First, take a look at your report from last
year; this may help jog your memory. Second, contact Bill Wagner your Regional Environmental Senior
Specialist, at 828.296.4705 or email to Bill.Wagner@ncdenr.gov. When you have completed your required
facility annual report, you will need to do the following:
1. Be sure to save a copy of the report on your computer, using "Save as..."
,2. Print a copy of the completed report. You can use the "Print Form" button found at the top of the first
page or use the program's print command. You need to be sure to sign the form. A copy of the form
must be sent to the County Manager of EACH county from which your facility received waste and a
copy must be sent to your Regional Environmental Senior Specialist (address below):

3. Submit a copy of the facility report electronically. This can be done by clicking the "Submit by Email"
button found at the top of the first page. Follow the instructions that appear on the screen. Essentially,
you are sending an email and attaching a spreadsheet containing the data from your report. Once your
form is submitted we will verify that it has been received by email.

New Permits: Solid Waste Facilities which have not previously been permitted by the Solid Waste Section; Permitted
Solid Waste Facilities proposing to expand the boundary of the facility for the purpose of expanding the permitted
activity; Permitted Solid Waste Facilities proposing to expand the boundary of a waste disposal unit; and permitted
Solid Waste Facilities seeking a substantial amendment* to a Solid Waste permit.
*"Substantial Amendment" means either: 1.) An increase of 10% or more in the population or the quantity of solid
waste to be disposed of in the sanitary landfill or the geographic area to be served by the sanitary landfill; or 2.) A
change in the categories of solid waste to be disposed of or any other change to the application for a permit or to the
permit for a sanitary landfill that the Commission or the Department determines to be substantial.
