The permit is for a five year period and is subject to review on or before 28 November 2006, as per 15A NCAC 13B .0201(c). This permit is for the operation of the new MSW bailer facility, the new white goods handling area, and the MSW transfer area adjacent to the existing facility. The existing facility will be used primarily for handling and sorting material from the county's Blue Bag program and as a back-up bailer in the event that the new facility is not operational. In addition to the Conditions of the Permit which address operational requirements, the facility must follow the Operations Plan that can be found in the approved application documents (document 95 of Attachment 1). Also enclosed with the permit is an Amendment to the Operations Plan prepared by Municipal Engineering Services Company, P.A. on Haywood County's behalf, dated 27 August 2001, that includes random screening procedures required at this facility since bailed material is not screened at the landfill. Ninety days prior to the expiration date of the Solid Waste Permit, the permittee shall notify the Solid Waste Section. Modifications, where necessary, shall be required in accordance with Rules in effect at the time of review. 2. The approved plan is described by Attachment 1, "List of Documents for Approved Plan". This facility may receive solid waste that is generated within Haywood County and shall dispose of said solid waste at the White Oak MSW Landfill (Permit No. 44-07) located on S.R. 1338 in Haywood County, North Carolina. 4. Prior to receiving waste at this facility, a Permit to Operate must be obtained. The requirements for this part of the Solid Waste Permit are described below in Construction Condition No. 9, The Conditions of Permit issued herein shall also be incorporated into the Permit to Operate. This permit is not transferable. 6. A copy of this permit and the approved plans shall be maintained at the facility. CONSTRUCTION: 7. This permit is for the construction of a mixed waste processing facility in accordance with the approved plans, prepared by Municipal Engineering Services Company dated 18 July 2000, and all pertinent rules and regulations. Any revisions or modifications to these plans shall be approved by the NC Solid Waste Section (the Section). All sedimentation and erosion control activities shall be conducted in accordance with the Sedimentation and Pollution Control Act, 15A NCAC 4, and all applicable permits shall be secured prior to construction. The following pre -operative conditions shall be met in order to obtain a permit to operate this facility: a. Construction shall be in accordance with the construction plan, and the conditions specified herein; b. Site inspection shall be made by a representative of the Division of Waste Management (DWM), Solid Waste Section, C. An operation and maintenance manual shall be submitted to the Section. The manual must incorporate the following: i. Detailed operational plans for the facility; ii. Maintenance and preventative maintenance requirements and schedules for plant equipment and instrumentation; iii. Control measures for mitigating wind blown litter, dust, odor, and vectors; iv. Emergency response plans and procedures for incidents including but not limited to fire, explosion, receipt of non -conforming wastes, etc.; V. List of staff positions required for operation of this facility and position descriptions; vi. Personnel safety requirements and established Worker Safety Plan; and vii. Proposed personnel training and instruction in areas of plant operation, plant and personal safety, maintenance and preventative maintenance plan, and emergency response plan. d) The permit number and the words, "No hazardous or liquid wastes accepted" shall be posted on a sign at the entrance and access controls shall be installed. e) Installation of the drains and leachate collection devices for the disposal of wash down water and leachate shall be operational and connected to the sanitary sewer line. 10. As -built drawings of the facility shall be submitted to the Section upon completion of construction. A fact sheet and a process flow diagram that summarizes actual equipment sizing and flow rates shall be included. OPERATION: 11. This facility is permitted to receive solid waste generated solely within the boundaries of Haywood County, North Carolina, as described in the approved plan, and as defined in 15A NCAC 13B .0101 (36), except that hazardous wastes, liquid wastes, medical waste, sludges, asbestos, and radioactive wastes are prohibited. The service area shall not be expanded without prior written approval from the DWM. This facility shall be designed, operated, and maintained in a manner so as to prevent the creation of a nuisance, potential health hazard, or a potential fire hazard. As necessary, effective vector control measures shall be applied to control flies, rodents, insects or vermin. 13. Waste shall not be stored on the "tipping floor" after operating hours. Waste can be stored in transfer trailers, at the facility with covers in place, after operating hours, but no longer than 48 hours. 14. The tipping floor and transfer trailer loading area shall be maintained in a clean, sanitary condition after operating hours in accordance with the approved Operational Plan. 15. Any water which comes into contact with solid waste shall be contained on -site or disposed of in accordance with the approved plan, the conditions specified herein, all pertinent rules and regulations. 16. Based on operating performance, the Conditions of Permit for the facility are subject to revision. 17. The storage period for recovered materials shall not exceed one year. 18. Adequate equipment for fire control shall be available and shall be tested as necessary to assure its proper operation in time of emergency. The facility shall meet all applicable NFPA codes. 19. Appropriate measures shall be applied to confine wind blown material on the property. 20. Non-processible/non-conforming wastes including, but not limited to construction and demolition debris, land -clearing and inert debris, white goods, lead acid batteries, etc., shall be segregated from the process waste stream and shall be managed, processed and/or disposed in accordance with the approved plan, and in accordance with all applicable local, state and federal rules. 21. All pertinent mixed waste processing facility personnel will receive training and supervision necessary to operate this facility. RECORD KEEPING AND REPORTING: 22. Fires and non -conforming waste incidents shall be reported to the Solid Waste Section within 24 hours, and a written notification submitted within 15 days. The permittee shall maintain records, compiled on a monthly basis, of the following information: a. total amount of waste (in tons) received at this facility for processing; b. total amount of waste (in tons) shipped from the facility for disposal; d. percentage reduction calculations; 24. On or before 01 August (or an earlier date as requested by the Solid Waste Section), and each year thereafter, the permittee shall report the amount of waste received (in tons) at this facility and disposed of in the Haywood County landfill to the Solid Waste Section (SWS) and to all counties from which waste was accepted, on forms prescribed by the Section. This report shall include the following information: a. The reporting period shall be for the previous year, beginning 01 July and ending on 30 June; b. The amount of waste received and landfilled in tons, compiled on a monthly basis, according to Condition 23 described above. 25. All records shall be maintained on -site and made available to the SWS upon request Amendment to the Haywood County Material Recovery Facility Revised Operation Plan August 27, 2001 The operator who is charging the conveyor continuously monitors the waste that is dumped on the tipping floor. He is continuously scanning the waste looking for items that cannot pass through the baler. These items will be such things as pressurized containers, paint cans or large objects that cannot fit into the baler without being broken down. The County is also going to install a video system where the operator in the booth of the baler can also view the tipping floor, baler and wire tier. The video system will be expanded so that the scale house operator may be able to view the waste in each truck as it enters the facility. The actual waste screening will be done on the tipping floor. The area of the tipping floor that will be designated for this purpose is on the southwest side outside of the normal flow of traffic. The truck to be screened will be randomly picked at the scale house and they will notify the loader operator via radio, which truck to screen. The loader operator will then have the truck dump its load in the designated area. The load will then be spread out with the loader and the operator will then manually sort through, using hand tools such as rakes and shovels. This will help to determine if there are any hazardous or unacceptable wastes.
