Life of Site Permit issued March 31, 2020 and will expire April 17, 2057. PURPOSE OF SITE VISIT: Comprehensive Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The facility is permitted to receive C&D waste generated in Harnett County. 2. The facility is fenced and is secured by a locking gate when it is not open for operation. 3. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact information as well as materials accepted at the facility. 4. The facility accepts waste Monday through Saturday from 7:30 am until 4:30 pm, with the exception of Wednesday from 7:30 am until 12:00 pm. 5. The working face was approaching the 1/2 acre size limit. Six (6) inches of earthen material should be properly applied once weekly or when the working face reaches 1/2 acre. The last date of cover documented on the cover log was November 4, 2021. The on -site attendant stated that he had been short staffed and had not been able to fully cover since November 4, 2021. He did state that he would have the working face covered on Friday, November 19. Ensure that the cover logs are up to date to properly document the cover applied. 6. At the time of this inspection, the waste type was identified to be construction and demolition waste. No unacceptable wastes were observed within the waste stream. 7. No areas of standing water were observed on the top of the CDLF Phase I and II. 8. No areas of erosion were observed along the side slopes of the CDLF Phase I and II. Mulch had been applied to the side slopes to help stabilize the soil until grasses could start growing. 9. The future CDLF Phase IV had been graded and the side slopes had been graded in preparation for use. Some erosion was noted on the side slopes. Ensure that the side slopes are repaired as needed. Also, contact the Section to schedule a Pre -Operation Inspection, prior to accepting waste in Phase IV. 10. The excess sediment previously identified in the sediment basin to the west of the CDLF had been cleaned out. However, the side walls were vertical with little to no vegetative cover. The side slopes of the sediment basin should be graded to a three horizontal to one vertical to allow for proper maintenance of the sediment basin. 11. Edge of Waste (EOW) markers were observed around the CDLF. 12. No woody or overgrown vegetation were observed on the side slopes of the CDLF. 13. A perimeter road has been established around the entire footprint of the landfill. 14. Windblown waste was observed on the side slopes of the CDLF. Mr. Chad Beane sent Ms. Thompson a photograph of the windblown waste on the CDLF being picked up on November 19, 2021. Ensure that the windblown waste is picked up by the end of the day. 15. Digital pictures were taken during the inspection. Yard Waste 16. The yard waste has been windrowed. As per the March 31, 2020 Permit to Operate, the windrows hand be a maximum height of 15 feet and the width of 50 feet in size at any time. During this inspection, the windrows were approaching the maximum height. It was recommended during the inspection that the yard waste pile be processed to reduce the size of the piles and allow for more space for the customers. 17. The waste stream was relatively clean with minimal unacceptable wastes. The on -site attendant removed a plastic swimming pool from the waste stream and properly disposed of in the transfer station. 18. No areas of standing water were observed within the yard waste area. 19. Continue to manage the yard waste area in accordance with the permit and approved operations plan.
