The Permit for Closure, issued April 8, 2015, allows for the collection of yard waste, scrap tires and white goods. The Permit for Closure states that the operations plan should be review every five years from the issuance of the Permit for Closure. PURPOSE OF SITE VISIT: Compliance inspection STATUS OF PAST NOTED VIOLATIONS: 1. 15A NCAC 13B .0505 (5)(b), "Surface water shall not be impounded over or in waste." RESOLVED: No areas of standing water were observed during this inspection. Previously identified low areas have been repaired and graded to promote drainage. OBSERVED VIOLATIONS: None Page 1 of 4 NORTH CAR()LINAD_E Q�� OepaNnent W Envimnmenbl W.I� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS 1. The facility is fenced and is secured by a locking gate when it is not open for operations. 2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact information as well as materials accepted at the facility. 3. The facility accepts yard waste, whitegoods, electronics, and scrap tires Monday through Friday from 0800 to 1630 and Saturday from 0800 to 1200. 4. The Permit for Closure requires the Operations Plan be reviewed every 5 years from the issuance of the Permit for Closure. Contact Donna Wilson at (919) 707-8255 or donna.wilson(&ncdenr.gov to discuss any possible new permits that will need to be obtained due to rule changes and the review of the Operations Plan. MSWLF (Closed) 5. The site had been mowed. 6. Some thorny woody vegetation was observed near the riprap for the slope drains. Mr. Falls stated that the woody vegetation would be cut, sprayed, and removed from the landfill cap by the forestry service. 7. Previously identified areas of settling or subsidence had been repaired and graded to promote drainage of the areas. 8. Edge of waste markers were observed around the Closed MSWLF. Yard Waste 9. No standing water was observed in the yard waste area. The area had been graded to promote drainage away from the yard waste storage piles. 10. No unacceptable waste was observed within the yard waste area. Continue to monitor incoming loads and remove any unacceptable waste as needed. 11. Mr. Falls stated that the leaves are stored separately from the branches. Mr. Falls also stated that he aerates and takes the temperatures of the leaf piles for composting purposes. 12. Mr. Falls stated that the branches had been recently ground and removed from the facility. 13. The yard waste piles were accessible on all sides. White Goods and Scrap Tire Area 14. The large concrete pad for the white goods and scrap tires was in good condition and properly sloped to prevent ponding water. 15. The white goods were upright and neatly stored on the concrete pad. The refrigerants are removed by the contractor. 16. The electronics will be palletized and wrapped prior to transportation to the recycler. 17. Scrap tires are stored at the opposite end of the concrete pad from the white goods. Mr. Falls stated that the scrap tires are picked up once or twice a week. 18.
