You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $5,000 per day may be assessed for each violation of the
Solid Waste Statute or Regulations. If the violation(s) noted here continue, you may be subject to enforcement actions including penalties, injunction
from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve
compliance with the North Carolina Solid Waste Management Act and Rules.
STATUS OF PAST NOTED VIOLATIONS (List all noted last audit):
No violations
OTHER COMMENTS /SUGGESTIONS:
1. Truck loading bay being kept clean, concrete pad for the white goods nearly full, no blown waste.
2. Tipping floor is showing damage — re -bar is starting to show through concrete, a small area of the harden surface is
beginning to breaking up.
3. Glass collection area OK. Bins of green. clear, and brown, loader is being used to crush glass.
4. Overall operation good.

2. Concrete pad for the white goods nearly full.
3. No waste is to be left on the tipping floor at the end of the work day. Waste can be stored in trailers if covered.
4. Tipping floor is showing damage — re -bar is starting to show through concrete, a small area of the harden surface is beginning

6. Area behind station cleaned up of vehicle storage.
7. Overall operation good.

PURPOSE OF AUDIT: Comprehensive audit
NOTICE OF VIOLATION(S) (citation and explanation): NC Administrative Code 13B Section 0402 (6) states:
Equipment shall be provided in the storage and charging areas and elsewhere as needed or as may be
required in order to maintain the facility in a sanitary condition.
Avety County is in violation of 13B .0402(6) in that Avery County has waste that is being left on the tipping floor
after the facility has closed for the day, leaving the facility in an unsanitary condition.
To correct this violation and be in compliance equipment such as a trailer or rolloff needs to be available to store the waste
removed from the floor at the end of the day. Waste can be stored in a trailer or rolloff when covered for up to 48 hours.
You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $5,000 per day may be assessed for each

STATUS OF PAST NOTED VIOLATIONS (List all noted last audit):
Violation of Conditions of Permit Which states: Waste shall not be stored on the "tipping floor" after operating hours. Waste can
be stored in transfer trailers, at the facility with covers in place, after operating hours, but no longer than 48 Ins. Approximately '/2
load of waste was being left on the tipping floor awaiting trailers after the facility has closed for the day. Avery Co. is providing
trailers in the afternoon as needed to comply with the requirements. Violation has been corrected
