You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $5,000 per day may be assessed for each violation of the
Solid Waste Statute or Regulations. If the violation(s) noted here continue, you may be subject to enforcement actions including penalties, injunction from
operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance
with the North Carolina Solid Waste Management Act and Rules.
STATUS OF PAST NOTED VIOLATIONS (List all noted last audit):

OTHER COMMENTS /SUGGESTIONS:
1. Waste floor cleaned regularly, truck loading bay clean, tires loaded in a trailer, no blown waste.
2. Floor has wear damage - rebar is starting to show through concrete, area of the harden surface breaking up.
3. Scrap metal area OK, Glass collection area OK.

1 Truck loading bay being kept clean, new concrete pad for the white goods, no blown waste.
2. Tipping floor is showing damage — re -bar is starting to show through concrete, a small area of the harden surface is
beginning to breaking up. Some waste has fallen over the side push walls recently.
3. Scrap metal area has been moved behind TS while concrete pad is constructed area OK, Glass collection area OK.
