The Solid Waste Section approved Duke Energy's request for replacing one damaged piezometers (CLF21-13)
with a new piezometer (CLF21-13R) at the perimeter of Cell 5 for the Industrial Landfill No.1 (1812-INDUS-2008)
Phase 2 expansion in letter dated October 27, 2021. The piezometer was installed to support collection of
groundwater data.
Duke Energy is providing the boring log, well construction record, and well abandonment record for the piezometer
replacement. The piezometer installation and abandonment was conducted by S&ME on November 4, 2021.
These records were submitted separately to the NCDEQ Information Processing Unit.

Abandonment and Replacement of Damaged Well CLF21-13

Information Processing Unit:
S&ME, Inc. (S&ME) is submitting the enclosed completed and signed Well Construction Record associated with
the installation of well CLF21-13R (installed to replace damaged well CLF21-13), as well a boring log and well
construction diagram. A completed and signed Well Abandonment Record for the abandonment of damaged well
CLF21-13 is enclosed as well.
A North Carolina registered well driller installed well CLF21-13R on November 4, 2021 in accordance with 15A
NCAC 02C Standards. Well development was subsequently performed on November 5, 2021, meeting Duke
Energy's criteria of groundwater turbidity lower than 50 NTUs. CLF21-13 was abandoned by a North Carolina
registered well driller on November 5, 2021 in accordance with 15A NCAC 02C Standards and Duke Energy's
Guidance for Well Abandonments in North Carolina (EHS Work Standard, Revision 1).

gallons of water and 8 pounds of Quik-Grout
(sodium bentonite-based grout). This mix was pumped
into the botton of the well using 1 inch tremie pipe.
