or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle tag#YA131845, VIN# correct per the firm application. The tank appears structurally sound &
securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of
this vehicle per info on the firm application.
2. Reviewed the truck pumping logs – it appears that the required information is being maintained, however please
begin adding the town name to the physical location for each septage pump job.
3. Discussed that the firm permit renewal process is currently under way. Mr. Smale stated that he had attended a
2021 continuing education class.
