enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected vehicle tag#JM7962, VIN# correct per the firm application. The tank appears structurally sound &
securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of
this vehicle per info on the firm application – discussed that the phone number that is on the truck is listed in the
“operator info” section of the application/not under “firm info” on the first page.
2. Reviewed the truck pumping logs – it appears that the required information is being maintained.
3. Discussed that the firm permit renewal process is currently underway. A representative for this firm has
attended a 2021 continuing education class.
