ADDITIONAL COMMENTS
1. Inspected vehicle tag#YA137631, VIN# correct per the firm application. The tank appears structurally sound &
securely attached. Outlet covers were on and no drips/leaks were noted. The lettering is correct on both sides of
this vehicle per info on the firm application.
2. Reviewed the truck pumping log – overall it appears that the required information is being maintained, except
that the full physical location for each pump job needs to be recorded.
3. Discussed that the firm renewal process is currently underway. A representative for this firm has attended a
2021 continuing education class.
