The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected two vehicles – tag#RNL0247, VIN# correct per the firm application and tag#CAK8649,
VIN#1GB3KVCY5KF145187, 400 gallon capacity (which was not listed on the current firm application). The
tanks on both vehicles appeared structurally sound & securely attached. Outlet covers were on and no
drips/leaks were noted. The lettering was correct on both sides of both vehicles per info on the firm application.
2. The firm permit renewal process is currently in progress. Ensure that the correct tag & VIN numbers are listed
on the renewal application for all vehicles to be covered under the 2022 firm permit.
3. Reviewed the truck pumping logs & discussed the required information to be maintained for each septage pump
job performed by this firm. Ensure that the final disposal location (specific WWTP) is clearly recorded in the
truck pumping logs.
4. If a company representative has not yet attended a 2021 continuing education class to obtain the required 4
hours of firm training, then please register asap for one of the remaining class locations.
